Use this form to order the Question and and Answer Service (QAS) or the Student Answer Service (SAS) for an SAT® test you’ve recently taken or
registered for. These services are not available for SAT Subject Tests™ or students taking the digital SAT.
Provide today’s date at the top of this form and complete the information below. Include your test information, birth date, and name exactly as they appear
on your score report (or admission ticket), along with your current contact information. (Your personal information is kept confidential.) Mail your order
and payment no later than five months after your test date to P.O. Box 7503, London, KY 40742-7503. QAS/SAS reports are delivered to most test
takers as part of their online score reports. If you don’t see your report upon score release, the report should appear within two weeks. If you registered
for the SAT through the mail, you’ll receive paper materials 8 to 10 weeks after the scores for your test date become available online.

INFORMATION ABOUT QAS AND SAS
The Question-and-Answer Service (QAS) is a test-disclosure service that includes a copy of the test you took with a guide that gives correct answers
and scoring information. You also receive a report that lists the type and level of difficulty of each question, along with what your answer was, and whether
it was correct, incorrect, or omitted. QAS costs $18. On test day you may have answered questions in a different order than what you see in your report.
The Student Answer Service (SAS) provides a report with information about the type of questions on your test, their level of difficulty, and whether
your answers were correct, incorrect, or omitted. SAS costs $13.50.
Use the table below to determine whether to order QAS or SAS. For a makeup, DANTES, or alternate test date, order SAS.
