CONTRACT OF SERVICE
KNOW ALL MEN BY THESE PRESENTS:
This contract was made and entered by and between:
The Municipal Government of Pigcawayan, Cotabato, a Local Government Unit, with an office address at
Poblacion III, Pigcawayan, Cotabato herein represented by HON. JUANITO C. AGUSTIN, Municipal Mayor, hereinafter
referred to as the “First Party”.
-ANDDAYBREAK BAND, represented by ___________________________________ of legal age, Filipino, and with
residence address at Barangay Presbitero, Pigcawayan, Cotabato Province hereinafter referred to as “Second Party”;
-WITNESSETH1. That the First Party is in need of the services of the Second Party who shall render a two-hour entertainment on
the occasion of the Local Live Band as part of the celebration of the 69th Founding Anniversary: Opening Salvo;
2. That the representative of the Second Party hereby attests that he is not related within the fourth degree of
consanguinity or affinity to the First Party;
3. That in view hereof, the Second Party are hereby contracted to render their service for one (1) day on October
20,
2022,
5:00
o’clock
in
the
afternoon,
with
a
service
rate
of
________________________________________________________________________________________
chargeable against the LGU;
4. That the payment (____________________) excludes the transportation of the second party and however shall
be accommodated by the first party through the Municipal Disaster Risk Reduction and Management Office;
5. That the Second Party shall provide band equipment at a time and schedule to be agreed upon by both parties;
6. That it is understood that this contract does not create an employer-employee relationship between the First
Party and the Second Party;
7. That failure of the second party to deliver the required services on time shall make them liable for any damages.
8. That deviation from this agreed contract shall be made the negligible party liable for whatever corresponding
damages;
