The District of Dumarao will conduct a Reading Assessment for Grade 1 (Hiligaynon), Grade 2 (Filipino) and Grades 3,4,5 and 6 (English), and Numeracy Assessment to all (Grades 1-6) learners on February 14-16, 2023. 2. Likewise, monitor the school office and classrooms on the implementation of 5S (Sort, Set-in-Order, Shine, Standardize, Sustain) for a safe and orderly workplace. 3. The activity aims to: a. assess the reading and numeracy levels of Grades 1to 6 leaners; b. evaluate school heads’ and teachers’ levels of implementation of 5S; and c. help teachers and school heads plan for appropriate interventions. 4. The schedule, composition of Monitoring Teams and mechanics are found in Enclosure No. 1 and 2. 5. Monitoring Tool for 5S is found in Enclosure No.3 and 4. 6. Expenses incurred relative to the conduct of this activity shall be charged against School MOOE subject to the usual accounting and auditing rules and regulations. 7. It is expected that in the conduct of this activity, all involved must adhere to minimum health and safety protocols set by the Inter-Agency Task Force (IATF) on the Management of Emerging Infectious Diseases and the Department of Health (DOH). 8. The Subject Area Coordinators in Mother Tongue, Filipino, English and Math will prepare two sets of assessment tools. 2. Words will be taken from learning materials used in the three learning areas and basic fundamental operations for Mathematics 3. Assessors will be given an assessment tool in the district office, then return the assessment tools to the team leader after the schools are done. 4. Assessment tools will be different per team. 5. The assessor will set the mood of the class before starting the assessment. Be friendly to ease tension. 6. For READING TESTS, let the learner read the words by pointing randomly to the listed words. 7. If the learner can hardly read, give him/her time to read at least 30 seconds. 8. Grade 1 pupils will read Mother Tongue, Grade 2 will read Filipino while Grade 3 to 6 pupils will read English. 9. If the learner can read at least 5 out of 10 words he/she is considered as READER. 10. For NUMERACY TEST, each leaner will answer the ORAL TEST in Mathematics. Grade 1 and 2 pupils will answer addition and subtraction, Grade 3 will answer addition, subtraction and multiplication and Grade 4 to 6 will answer addition, subtraction, multiplication and division. 11. Learners can be cosidered NUMERATE if: a. Grade 1 and 2 - can answer at least 3 correct answers in every operation. b. Grade 3 and 4 - can answer at least 6 correct answers in every operation. c. Grade 5 and 6 - can answer at least 8 correct answers in every operation. 12. If the class has 100% readers with an attendance of 95% and above, the teacher will receive a certificate of recognition. Another certificate of recognition will be received by the teacher if he/she has 100% numerates with 95% and above attendance. 13. If all the pupils from Grades 1-6 are readers and numerates, the school head will receive a certificate of recognition for having a school with 100% readers and numerates 14. Submit reading and numeracy assessment results to the team leader or assistant team leader on the day of the assessment for immediate encoding. MECHANICS ON HOW TO EVALUATE SCHOOL’S ON ITS IMPLEMENTATION OF 5S 1. The district adapted the Monitoring and Evaluation (M&E) Tool of SDO Capiz and modified by Public Schools District Supervisor (PSDS). 2. The PSDS or Team Leader will evaluate the school head’s office and teacher’s classroom using the district modified M & E Tool. The M & E Tool is composed of 25 items for 5S, Sort, Set-in-Order, Shine, Standardize, Sustain, for a safe and orderly workplace. 4. The 5S are: Sort (eliminate that which is not needed) – the act of discarding all unnecessary and unrelated materials in a particular workplace. The number of needed items must be kept to its minimum to ensure that every item in the workplace is related to work resulting to effective use of space Set-In-Order (organize what remains after sorting) – placing everything into its designated place so that it can be accessed or retrieved, as well as returned quickly. Shine (clean and inspect the work area) – sweeping up the workplace. Maintaining cleanliness should be part of the daily work Standardized (write standard for 5S) -more or less translate to standardized clean-up. It consist of defining the stndards by which … Sustain (consistently apply the 5S standards) – 5. Safe and Orderly Workplace: a. anything needed hourly should be within easy reach on the desk; b. anything needed daily should be placed below the desk; c. anything needed weekly should be placed in cupboard/shelves; and d. anything needed only monthly, quarterly or yearly should be stored in a cabinet. 6. Each S has five statements and each will be scored 5, 4, 3, 2, 1, or 0. The score descriptions are: 5 = No deviation identified 4 = One deviation identified 3 = Two deviations identified 2 = Three deviations identified 1 = Four deviation identified 0 = More than four deviations identified. 7. All S scores will be added together to provide a Total Score for 5S. 8. A school Head’s total score will be rated as follows: 100 - 125 75 – 99 Below 75 - OUTSTANDING (strong 5S culture is already in place) - VERY SATISFACTORY (Solid 5S implementation but with room for improvement) - SATISFACTORY (Significant potential for improvement) 9. The PSDS or Team Leader will review the total score with the school head and the team member will review the total score with the teacher as appropriate. 10.
