CHAPTER 1
INTRODUCTION
The purpose of this manual is to demonstrate and explain how INDEXPRO INT’L SERVICES LIMITED
plans to implement its policies, procedures and targets.
The HSE-MS manual ensures that policies, objectives, organisational structure, responsibilities, standards,
processes, procedures, controls and resources, are defined and aligned to manage INDEXPRO INT’L
SERVICES LIMITED HSE risks and impacts, according to defined standards. In addition, it also ensures
that processes and procedures are in place to identify, assess and control HSE risks to agreed
performance standards and to recover from HSE incidents should they occur.
The natures of our operations present hazards which graduate from minor first-aid-injuries to the dangers of
large injuries and loss of life. The operations directly and also pose serious challenges to the Health of the
operators, Community Affairs, the Environment, and the Security of the life and properties, of even clients
and third parties.
These operations require our employees to undertake tasks, which, if not carried out under stringently
applied safety and health procedures and regulations, can expose them to risks of serious dimensions. The
employees have the right and privilege to know that the company has their wellbeing as its prime concern.
With our planned large and widely dispersed operations, it is imperative, if we were to assure uniformity
and competence in carrying out our HSE responsibilities. We in INDEXPRO INT’L SERVICES LIMITED
believe that a healthy and relaxed worker is an efficient worker. Employees are encouraged to have
absolute commitment to their jobs and always work as a team.
To ensure effective planning before embarking on any of our activity, INDEXPRO INT’L SERVICES
LIMITED shall ensure there is a supervisor at the work front. The supervisor shall ensure the work force is
aware of every detail of the job, familiarise themselves with the contingency and emergency procedure.
The supervisor is empowered to stop the work if there is any violation in HSE.
The company shall adopt stricter measure to ensure that all injuries to personnel and property are
completely eliminated or reduced to a minimum. All Health Environment and Security regulation and
legislation shall be complied to and implemented.
CHAPTER 2
LEADERSHIP AND COMMITMENT
The leadership and commitment expected of all INDEXPRO INT’L SERVICES LIMITED sectional heads
and line managers to demonstrate commitment in HSE are as follows:

Belief in the company’s ability to continually improve HSE performance.

Empowering and encouraging staff at all levels to take personal responsibility for and take a leadership
role in HSE issues relevant to their professional work, their work environment, and host communities.

INDEXPRO INT’L SERVICES LIMITED will set clear objectives and targets for HSE and reviewing
performance at regular intervals.

The company INDEXPRO INT’L SERVICES LIMITED will ensure and create a non-blame working
environment.
INDEXPRO INT’L SERVICES LIMITED sectional heads and line managers shall lead by example,
motivate employees under them to adopt high HSE standards, provide resources to identify eliminate or
reduce and manage HSE risks.
This is shown as follows:

INDEXPRO INT’L SERVICES LIMITED will formulate and periodically review strategic
objectives.

Finally he shall perform management reviews.

Leading medium and lower potential incident investigations.

Manage INDEXPRO INT’L SERVICES LIMITED sub-contractors HSE.

Implementing the INDEXPRO INT’L SERVICES LIMITED sectional Management System.

Leading sectional HSE Meetings.

Participate in management site audit inspection schedule.
CHAPTER 3
POLICY AND STRATEGIC OBJECTIVES
INDEXPRO INT’L SERVICES LIMITED HSE policies are divided into three 1st, 2nd and 3rd tiers. The 1st
covers the HSE policies statement, while the 2nd and 3rd covers specific aspects of HSE Management,
which is applicable to INDEXPRO INT’L SERVICES LIMITED.
These policies address specific risk that can have negative impact on lives, assets, environment and the
health of the workforce, sub-contractors, host communities, and other third parties that might be affected by
company activities.
POLICY STATEMENT
INDEXPRO INT’L SERVICES LIMITED shall carry out its activities in such a way as to:
1) Preserve the health, safety and security of the employees and all members of public who may be
affected by our operations.
2) Minimise the impact of our activities on the environment and ensure that the environment is kept in a
better state than we met it.
3) Ensure standard at all times and work in harmony with the host community.
All such activities that cannot be carried out in accordance with this policy will be suspended until
acceptable solutions are found.
Line management is responsible for the full implementation of this policy.

HEALTH, SAFETY AND ENVIRONMENT POLICY STATEMENT
The company gives priority attention to health and safety of everyone directly or indirectly affected by its
activities and due regard to the environment. Therefore, we shall plan and conduct our activities in such a
way as to:

Avoid injuries to our staff and sub-contractors through proper job planning. Effective controls of
occupational health hazards and prompt medical attention.

Minimise the impact of our activities on the environment.
In order to achieve these objectives, every employee must plan and execute his job in accordance with this
policy. No job should start unless all precautions have been taken. All jobs started must be suspended
when it is believed that is not safe to work. Management must strive to provide a safe work environment at
all times.
Resources shall be made available for the training of staff. Health, Safety and Environment shall be
managed in the same way as production and cost control.
HSE performance and contribution shall be used as a tool to judge the effectiveness and performance of
every staff. The same standard shall also apply to our contractors and sub-contractors.

INDEXPRO INT’L SERVICES LIMITED shall continue to identify with the needs and aspirations of her
clients and host community in the execution of its operations.
The company shall also ensure the building and sustenance of a harmonious relationship with her client
and host community. She shall exhaust every known peaceful strategy to resolve conflict in case of any
disagreement.
In order to pursue mutually beneficial relation with Host Company, we shall:

Carry out Environmental Impact Assessments and Environmental Evaluation Reviews in relation to
all aspects of the natural and social environment that may affect, or may be affected by its activities

SMOKING POLICY
Smoking is injurious to health. Therefore, the management of INDEXPRO COMPLETELY discourages
smoking of any sorts, inhalation or chewing of tobacco but prohibits smoking of any sorts, inhalation or
chewing of Indian hemp in all our work sites.
However, INDEXPRO has a designated ‘’CIGARETTE ONLY’’ smoking area for visitors and workers who
chose to indulge.

INDEXPRO INT’L SERVICES LIMITED SEAT BELT POLICY
Our seat belt policy is geared towards achieving a non-fatal impact on road traffic accidents on our drivers
and passengers.
We shall ensure:
a)
All company vehicles must be fitted with functional seat belts at every available seating
position.
b)
Occupants in our moving vehicles must wear seat belts.
c)
A vehicle, which does not meet with the above requirement, is not fit for our use and is also
not given access into our premises.

PERSONAL PROTECTIVE EQUIPMENT (PPE) POLICY
It is the policy of INDEXPRO INT’L SERVICES LIMITED to:
To conduct a hazard assessment to identify the appropriate PPE for each work activity.

All supervisors will perform a job-specific PPE orientation to inform their subordinates of the PPE
requirements resulting from the hazard assessment.

All employees and visitors must wear approved head protection in designated areas at site.

Head protection will also be won anytime there is a danger of falling objects or there is possibility
that a head injury may occur while performing duties.

Where the use of PPE is unavoidable, to supply the PPE at no cost to those requiring it.

It is an offence for any employee of INDEXPRO INT’L SERVICES LIMITED to be under the influence of
alcohol or in possession of, or under influence of any non-prescribed drug (cocaine, amphetamine (speed),
marijuana) or other illegal alcohol/drugs on our work site.
Any employee found under the influence of drug or alcohol in any of our worksite operations will have
his/her appointment terminated immediately.
Only qualified medical practitioners are authorised to administer drugs to our workers. Only WHO approved
list for First Aid is allowed on site.

Due to high rate of insecurity at worksite, INDEXPRO INT’L SERVICES LIMITED has a policy that:

Work shall not commence any day unless site is certified SAFE and any risk associated
with work taken care of.

All workers shall be provided with identity cards.

Only visitors with proper identification shall be allowed entry to

It is the policy of INDEXPRO INT’L SERVICES LIMITED to segregate all wastes resulting from her
activities, office, work shop and work site, thereafter dispose appropriately as stated in our waste
management plan.
Work sites and temporary structures shall be kept tidy.
Wastes, disused parts and surplus materials shall be removed daily to approved dumpsite, reused where
necessary and base office respectively.

The Policy on driving any INDEXPRO INT’L SERVICES LIMITED vehicle be light or heavy duty vehicles
are listed as follows:
All drivers will have undergone a record of road tested and defensive course before driving for the
company.
Unauthorised driving of company vehicles is not allowed.
Approved drivers are to drive vehicles within the class (grade) of vehicles for which they are
approved.
All drivers to be approved by the Safety Manager
Seat belts must be worn always by drivers and all persons in any company vehicle
No persons are to be carried in the back of company pick up vehicles.
INDEXPRO INT’L SERVICES LIMITED vehicles are to be used for only INDEXPRO INT’L
SERVICES LIMITED business, unless the assigned employee has prior written authorisation.
Driving under the effects of alcohol is prohibited.
All state and Federal driving regulations shall be observed.
INDEXPRO INT’L SERVICES LIMITED’ Policy on speed limits must be followed.
No NIGHT DRIVING (1900 to 0600 hours) is allowed.
All vehicles not assigned by the Area Manager/Logistics Manager must be left at the base.
Accidents, no matter how small must be reported.
All accidents must be investigated.

It is INDEXPRO INT’L SERVICES LIMITED’ Policy to:
use chemicals only where operationally necessary

consider the HSE aspects together with commercial and process performance attributes in selecting
chemicals for use, with the aim of choosing the most suitable

transport, store, use and dispose of all chemicals in accordance with statutory requirements and in
a safe and environmentally responsible manner

provide Safe Handling of Chemicals (SHOC) cards for all chemicals used in the company

IMPLEMENTATION AND DISSEMINATION OF POLICIES AND OBJECTIVES
INDEXPRO INT’L SERVICES LIMITED will create awareness of its HSE policies at all level of staff and
they are held accountable for its implementation within the area of operations for which they are
responsible.
Copies of the policies will be made available to all INDEXPRO INT’L SERVICES LIMITED’ staff and subcontractors through the following means:
 All new and old INDEXPRO INT’L SERVICES LIMITED’ staff are given a personal copy of the
policies after an open discussion of how the content relate to his/her function.
 All company HSE policies will be pasted on the notice board.
To ensure all sub-contractors are fully aware of INDEXPRO INT’L SERVICES LIMITED’ HSE policies, the
following action is required:
All sub-contractors are made to sign an HSE commitment and procedure on given a job.

They will be encouraged to have corporate or local policies similar to those of INDEXPRO INT’L
SERVICES LIMITED.

Sub-contractors are required to ensure similar dissemination of their own policies to all their staff.

INDEXPRO INT’L SERVICES LIMITED will discuss all policies with sub-contractor before
commencement of work.

These policies will be displayed at conspicuous location around the worksite.
CHAPTER 4
ORGANISATION, RESPONSIBILITIES, RESOURCES, STANDARDS AND DOCUMENTATION
INDEXPRO INT’L SERVICES LIMITED is made up of three major sections Administration, HSE / Projects,
and Procurement. These sections are headed by various line managers.
Responsibility for HSE management is spread throughout INDEXPRO INT’L SERVICES LIMITED giving
everybody a role to play within their area of direct influence. Responsibilities cascade down from the
Managing Director through the various line managers and ultimately, to individuals as shown in the
organogram.
ROLES AND RESPONSIBILITIES OF KEY PERSONNEL
MANAGING DIRECTOR
The Managing Director must have a minimum of a bachelor degree or its equivalent in any field of study
and is charged with the following responsibilities:
Directing the safety policy and programme implementation.
Co-ordination, planning and development of in-house procedures.
To ensure compliance with statutory and clients' safety
rules/regulations.
Ensure implementation of all planned safety training for all employees.
Provision/procurement of all necessary personal protective/equipment through budgetary approval.
Co-ordinate all line safety activities by taking charge of the monthly management safety committee.
Encourage employee participation in the safety effort through establishment of a two-way
communication channel.
Regularly monitor employee driving habits to ensure compliance with Company road and industrial
safety regulations.
PROJECT MANAGER
The Project Manager must have a minimum of HND or its equivalent in any field of Engineering and
Environment Studies and shall assume the point role for all safety matters affecting the technical functions
including the following:
take charge of company affairs in the absence of the Managing Director
participate in the general planning and execution of the project
liaise with client and statutory bodies
carry out the job HSE audit
ensure issuance of PPE
participate in the investigation of accidents
develops procedures/implementation and follow-up
ensure that company HSE policy is strictly adhered to
Carry out spot checks in all work sites to monitor field safety practices.
Co-ordinate all field safety meetings/trainings, including off-the -job, job safety instruction, and all
other trainings as may be deemed necessary for the safety of men, operations, materials, and
environment.
Direct responsibility to the Managing Director for all project safety practices.
Development of procedures/practices, implementation and follow-ups.
Ensure issuance of personal protective equipment and encourage use.
Join in the investigation of all accident to determine immediate and remote causes.
Periodic auditing of project safety compliance in conjunction with the Safety Officer and Site
Engineer.
SITE ENGINEER
The site Engineer must have a minimum of HND in any field of Engineering and is charge with the following
responsibilities:
Overall responsibility for safety at his site.
Head the site safety committee.
Investigate all accidents occurring therein.
Oversee the site safety training operations.
Conduct regular site inspections to ensure a high standard of housekeeping.
Encourage the use of personal protective equipment by effective training and monitoring.
Ensure compliance with all safety rules and regulations through constant communication with site
crew.
Organize site safety drills as planned.
Join in the development of new procedures.
ADMINISTRATIVE MANAGER
Administrative Manager must have a minimum of B.Sc. in a any field of study with the working experience
of 4 years in administration and is charged with the following duties:
Co-ordinate all office HSE efforts
Serve as Secretary during the Management Safety Committee (MSC)
Give administrative support to the General Manager in development of strategies for effective HSE
management.
Draw up procedures for counselling accident victims.
Organize and schedule office block safety drills and emergency preparedness
Monitor first-aid administration and replenishment as necessary.
Monitor safety training programme availability locally and bring it to the attention of General
Manager for consideration
Monitor employee's driving habits as per schedule to evaluate compliance with the company rules
and the Highway Code.
Establish a safety information system to ensure accurate documentation and retrieval of past
events.
Maintain and update a safety statistics gathering system to aid yearly evaluation.
HSE MANAGER
The HSE Manager must have a minimum of B. Sc in any field of study with training and certificate in HSE
program and is charged with the following duties:

Shall seek to initiate or develop safety measures in all activities of the company
Shall develop and sustain a high level of safety consciousness among the work force.
Shall ensure that SHE policies are complied with throughout the organization
HSE administration particularly in the Head office.
Organizes HSE meetings
Shall ensure that any job considered unsafe is stopped or never started at all.
Shall stop any activity that believes may cause an accident.
Shall prepare monthly and yearly SHE report.
Shall assess risks of any activities and communicate to personnel as well as suggest preventive
measures.
Reporting to the MD/CEO

SAFETY OFFICER
Safety Officer is a key figure in the accident prevention effort. He must have a minimum of OND in any
field of Engineering and must have trained on safety. He should also possess similar qualities to a trainer,
since he can only get his job done through the safety skills and proficiency of others.
He is responsible for:
Leading the location drive to promote the safety Awareness and Prevention Plan
Safety administration, i.e. keeping comprehensive and detailed safety records of:
People
Equipment
Safety audits
Safety Training
Compliance with all government and other statutory safety regulations.
Active management and control of safety aspects within the site
Conducting investigation into all accidents and near misses

HOW TO MANAGE STAFF COMPETENCE ASSURANCE
INDEXPRO INT’L SERVICES LIMITED competence assurance is based on the requirement that, at all
times; all work roles are performed by people with sufficient motivation and competence to discharge
duties. Competence assurance process is a key component of the management system. It is required to
ensure, that staff have the required levels of skills, knowledge, and awareness to perform their roles
adequately and safely to specified standards.
It is mandatory on all workers to sign the attendance register on reporting for work. During work, if there is
cause for anyone to leave the site he/she must log his name in the movement register. This is to enable
management monitor access in and out of site.
The key personnel on this work are identified as the Site Engineer, Safety officer and Site Nurse.
Provisions are made for call- off key personnel when they are absent from site.
Where an Employee fills an HSE critical position and is planned to be moved to another position or go on
leave, it is the sectional head’s responsibility to demonstrate, via the competence assurance process, that
the new incumbent has had proper training, experience and job hand over.
In INDEXPRO INT’L SERVICES LIMITED it is the responsibility of all employees on to manage safety
though the safety officer will be the driving force and he is to report directly to Managing Director.

Communication is a vital tool for an effective management of HSE at the work site. In INDEXPRO INT’L

Monthly HSES meetings are held and it is chaired by INDEXPRO INT’L SERVICES LIMITED Managing
Director. During these meetings, HSE issues are the item on the agenda. The focus is generally on
cascading HSE information from management meeting, HSE performance / awareness presentation and
incident review if any during the month.
Quarterly Sub Contractors forum
Quarterly HSE meetings are held with all sub-contractors that have operation with us. The meeting is
chaired by the Project Manager or Sectional head. Senior representative from the sub-contractor are to be

Tool Box Talk
Before commencement of work each morning, the safety officer will assemble all the workers for a short
briefing for 10-15minutes. These briefing which shall be documented, will review the scope of activities for
the day and highlight both hazards and precautions necessary to deal with them. At the end questions are
asked for further clarification.
SUB-CONTRACTOR SAFETY
Contractors and sub-contractors performing work for INDEXPRO INT’L SERVICES LIMITED shall conduct
their work according to safe and healthful operating practices. They shall follow all applicable safety and
health rules and regulations as specified in our HSE handbook.
Sub-contractors are required to develop its management system in the context of its own organization, but
compatible with the INDEXPRO INT’L SERVICES LIMITED management system. Since sub-contractor
companies differ in size, complexity and orientation, It is the duty of every sub-contractor to understand the
INDEXPRO INT’L SERVICES LIMITED HSE Management System.
The Sub-contractors HSE Management System should include:

HSE DISCIPLINARY (PUNITIVE) MEASURE
To ensure that every employee takes HSE very seriously, the management shall deem it necessary to
adopt some disciplinary actions to act as check on performance and behavior.
A member of the work force who violates the HSE rules and regulation shall in the first instance, be warned
orally by the site-supervisor or the HSE officer. Where he continues, he shall be issued with a written
warning or query to be answered within 24 hours. Thereafter, he shall be suspended, dismissed or
terminated based on the severity of the offence.
NOTE: - All suspension notice shall be observed without salary or wages.
STAFF COMPETENCE
The Safety officer will ensure that all workers are in their best mood to work. Any worker detected to be
mentally, physically or emotionally ill or troubled should be asked to stop work as his state of condition is
prone to unsafe act or accident.
The Safety officer must see at all times that before any worker is mobilized to site to commence work he is
first taken to the retainer ship clinic to be properly examined and certified fit to work. He will also ensure
that no worker is under the influence of alcohol or drugs, and if any is found ensure that such people should
stay away from site.
TRAINING
Management understands that a trained work force is a valuable asset. To this end, the contract manager
shall liaise with appropriate departments to ensure that workforce is trained on

HSE STANDARD
The safety and wellbeing of each employee is our primary concern.
Therefore, any employee who disregards safety regulations, practice and rules, thus endangering
himself/herself or other employees will be subject to disciplinary action including termination. Working
safely is a condition of employment.
Controlled areas
A controlled area as designated by the supervisor is that area in which he/she is responsible for all
movements and operations of personnel and equipment. Certain sections may be designated a hazardous
area, and as such could be subject to special rules, regulations and restrictions required to promote safe
operations. Controlled areas shall be well defined (Cordoned off if applicable), and all persons whether
working or visiting shall abide by the safety rules of the controlled work area.
Visitors
The Safety Officer, who has a thorough knowledge of safety rules and procedures, must accompany all
visitors, to controlled areas after the visitor has been inducted. He/She must comply with the safety
regulations and requirements deemed appropriate by operations.
Safety Induction for New Workers
The safety officer will conduct an induction briefing for all the workers at the inception of the work. The
induction will center on the hazards that would be encountered while performing the work. In addition, he
will talk on actions taken in the event of emergency.
Smoking
“No Smoking” notices shall be prominently displayed in restricted sections of a controlled area. Single
action (disposable) lighters are NOT allowed at our customers’ locations.
Monthly Safety Meeting and Drills
Monthly safety meeting and drills shall be scheduled and conducted by the safety department, on a monthly
basis (at a minimum). Attendance is mandatory unless excused by a supervisor.
The minutes of each meeting and names of those in attendance shall be documented.

Each employee has personal responsibility to perform his/her job in a safe manner. The contributing factors
of most accidents are thoughtlessness, inattention, haste, or lack of knowledge. By receiving the proper
training, using good judgment, following safety rules and being alert, accidents and potential injuries can be
avoided. Safety is a full-time responsibility of everyone at the work site.
Alcohol & Drugs
Employees have a responsibility to report to work fit for duty. Any medications prescribed by your
physician or which you are taking may have an effect on your ability to perform your job safely. Report any
medications being taken to your supervisor.
HSEMS DOCUMENTATION AND ITS CONTROL
HSEMS record keeping means that orderly arrangements and placement of our health, safety and
environment and security records. Good record keeping implies that there is a place for all records and
every record is in place. This reduces the accident rates, fire hazards, operating cost and wastage.

PERMIT TO WORK (PTW)
INDEXPRO INT’L SERVICES LIMITED uses permit to work (PTW) for all sensitive and non routine
activities of our projects. Typical activities for which permit to work are applied are:- hot work, excavation
,entry into confined spaces, work height, road works, handling of chemicals, clerical work etc.
HAZARD AND EFFECTS MANAGEMENT PROCESS
Risk levels are classified as ‘tolerable’ LOW or ‘tolerable but should be reduced to MIEDIUM and
‘intolerable’ HIGH. The term ‘As Low As Reasonably Practicable’ [ALARP] has been adopted to address
the fact that at a certain point the effort needed to further reduce the risk is simply not worth the benefit.
Once a risk is deemed tolerable it is continuously managed through proper controls and normal precaution.

Job Hazard Analysis (JHA)
Each step or activity should briefly describe what is done. Each activity should be listed on the form in the
order it is accomplished.
Avoid the common errors of making the breakdown so detailed that an unnecessarily large number of steps
result, or making the job break down so general that basic steps are omitted. If a large number of steps
result from the analysis (over 15), you should consider breaking that job into more than one JHA.
It is important that the JHA accurately describes the work.
Keep in mind that any JHA that is too broad is also a poor JHA.
Identifying the Hazards Associated with each Job Step
After all basic steps of the operation have been listed; next we need to examine each job step to IDENTIFY
HAZARDS ASSOCIATED WITH EACH JOB STEP. The purpose is to identify and list the hazards, which
are possible in each step of the job. Some hazards are more likely to occur than others, and some are
more likely to produce serious injuries than others. Consider all reasonable possibilities when identifying
hazards.
To make this task manageable we will work with basic types of accidents. The question we will ask
ourselves is, “Could any of these accident types or hazards inflict injury to a worker?”
The second JHA form is a list of potential hazards that are not all encompassing, but can be used to assist
in the thought process.
Controlling each Hazard
The next part of the JHA process is to develop safe practices to eliminate or reduce potential accidents or
hazards that have been identified for the job step:
Less Hazardous Way to do the Job.
Can a less hazardous way to do the job be found? Can an engineering revision take place to make the job
or work area safer? Is there a better way to do the job? This requires determining the work goal and then
analyzing various ways to reach the goal to see which way is safest. Is the work saving tools and
equipment available that will make the job safer?
Physical Conditions
Can the physical conditions that created the hazard be changed? Physical conditions may be tools,
materials, and equipment that may not be right for the job. These conditions can be corrected by
engineering revisions, administrative revisions, or a combination of both.
Change of Job Procedures
If hazards cannot be engineered out of the job, can the job procedure be changed? Be careful here
because changes in job procedures to help eliminate the hazards must be carefully studied. If the job
changes are too difficult, long, or uncomfortable, then the
employee will take risks or shortcuts to avoid these procedures. Caution must be exercised when changing
job procedures to avoid creating additional hazards. For details see Job Hazard analysis table below.
CHEMICAL MANAGEMENT GUIDE
When there is a potential exposure to chemicals through inhalation, ingestion, skin absorption, or injection,
personal protective equipment shall be utilized.
Personal Protective Equipment (PPE) is any barrier worn on the body to reduce or eliminate a hazard.
INDEXPRO INT’L SERVICES LIMITED will conduct a hazard assessment to identify the appropriate PPE
for each work activity.
Supervisors will perform a job specific orientation to inform his subordinate of the PPE requirements
resulting from the hazard assessment.

TIPS ON SAFE HANDLING OF CHEMICALS
 Many chemical vapours travel close to the ground and accumulate in areas. Be aware of potential
accumulation areas.
 Some chemicals are detectable by odour; however, do not rely on your sense of smell. Detection
devices may be required to determine the presence of some chemicals. Many chemicals have
immediate exposure effects.
 If there are any questions pertaining to chemical, refer to the MSDS for the chemical and / or your
supervisor.

CHAPTER 5
PLANING AND PROCEDURES
EMPLOYEE BEHAVIOR
Each employee has a personal responsibility to perform his or her job in a safe, professional manner.
 Horseplay, such as practical jokes, wrestling, fighting, hazing, etc., will not be tolerated and is prohibited.
 When working together, protect not only yourself, but avoid actions that might cause problems for your
fellow employee.
 Smoke only in designated or approved areas.
 Long hair and/ or jewelry can be a hazard around machines with rotating and moving parts.
ENVIRONMENTAL MANAGEMENT
INDEXPRO INT’L SERVICES LIMITED desires to be an environmentally friendly company. We are
concerned not only with the health and safety of those on our premises but also with those away from our
premises who could be affected by our operations. It is important that any waste, which we generate, is
disposed of correctly so it does not create an unacceptable risk to others and the environment. Your
responsibilities are to follow established company rules pertaining to all disposal operations.



Report any leaks or spills to your supervisor immediately.
All containers for waste disposal must be labeled so they may be disposed of properly.
Do not dispose of waste substances into sewers or surface water drains unless it has previously
been approved to do such in agreement with established company procedures. If you are unfamiliar
with proper disposal procedures, seek guidance from your supervisor prior to disposal activities.
BASIC HSE RULES FOR ALL JOBS
 Know how to do the job.
 Examine new assignments to determine possible safety hazards.
 Help new or inexperienced employees by teaching them how to perform the job in the correct
manner. Point out potential hazard.
 Use Job Hazard Analysis (JHA) to teach new or inexperienced employees job safety.
 If you are unsure about the safety of any operation, ask your supervisor.
 Take action to correct unsafe conditions in your area.
 Respect fellow employees. Warn them before proceeding with any operation that will affect their
safety.
 Know local rules for emergency situations. A good practice is to think out in advance what you
should do in various emergencies.
 Use safe working procedures at all times.
 Use stairs, walks, and gangways provided – do not take shortcuts, do not jump from elevated
places. Running is prohibited, except for emergency purposes.

Employees shall not wear any jewelry (other than wristwatches) while in areas requiring the use of
hand tools, power tools, machinery and or equipment.

Housekeeping is the responsibility of every employee. Upon the completion of each job, left over
material and trash shall be removed. Tools and equipment shall be put in their proper place. Used
or replaced parts shall be disposed of properly.

Keep your work area clear of spills, i.e., oil, chemicals etc.


Manholes, open hatches, loose grating and miscellaneous openings create hazards. Always keep
openings covered or place guards or barriers around them.

Dispose of waste material in designated containers.

Report any cleanup problems that you cannot take care of yourself, but first secure the area.

Use only INDEXPRO INT’L SERVICES LIMITED approved cleaners for cleaning purpose.

Use approved soaps and cleaners provided for cleaning skill. Do not use solvents.

Change oil-soaked or contaminated clothing. It may cause skin irritation and is a fire hazard.

Learn how to lift properly and obtain help when moving or handling objects that are too heavy to
move safely.

Use proper tools and equipment maintained in good working condition. Use the right tool for the
job.

Dress properly with your PPE before job


INDEXPRO INT’L SERVICES LIMITED approved first aid kits and first aid-trained personnel shall
be available on all work locations.

Walking on tank tops is prohibited except in designated walking areas or where fall-protection is
provided.

When operating in the vicinity of electric overhead lines, assume that all lines are energized. Never
bring any conductive object (crane boom, back hoe, etc) closer to unguarded, energized overhead
lines than the distances given below:
CHAPTER 6
EMERGENCY RESPONSE
Even with the most stringent preparations, emergencies still happen. It is the duty of each employee to be
ever alert; to recognize emergencies, and to be prepared to take the proper action when an emergency
occurs.
Some emergencies require immediate action; others require studied action. Good judgment applied in
each case will indicate the proper action necessary. This Emergency Plan (EP) is in place to ensure
employee safety from fire and other emergencies. It provides a written document detailing the actions and
procedures to be followed in case of emergency.
SAFETY DRILLS
Safety meeting shall be scheduled and conducted by the Department, or supervisor, on a monthly basis (at
a minimum). Attendance is mandatory unless excused by a supervisor.
The minutes of each meeting and names of those in attendance shall be documented.
SITE EMERGENCY PROCEDURE
The following procedure shall be followed strictly in the event of any emergency.
In the event of an accident on site every employee should:
Keep cool and avoid panic

Alert other workers around the scene of the accident.

Evacuate victim to retainer ship clinic to receive medical attention.

EMERGENCY/CONTINGENCY PLAN
In order to effectively handle emergencies, this plan and procedure shall be applied. To be able to do this
satisfactorily, a good knowledge of foreseeable or likely emergencies is required. Our assessment of the
whole job reveals three possible emergency situations these are work injury, fire out break, electric shocks
and assault.
WORK INJURY EMERGENCY
First Aider will approach victim and render first aid treatment using available facility (first aid box) provided.
Where further treatment is required, victim shall immediately be evacuated to our company retainership
clinic.
In serious cases where MEDEVAC facility would be needed, a call shall be made to clinic or section head
and he will be given essential information about the accident.
The company’s Managing Director and Project Engineer shall equally be informed.
FIRE OUT BREAK EMERGENCY
In the event of a small fire, it would be put out using portable fire extinguishers. Big fires require more
technical approach and the trained fire fighter shall be invited to deal with the situation. Equally a call shall
be made to inform the Project Engineer, Fire Dept, and retainership clinic Base office.
In case of fire workers are advised to act as follows.
Raise alarm to warn people in the vicinity
Shut off power supply and remove any combustible materials near the fire
Fight fire with appropriate firefighting equipment if it is safe to do so.
Inform supervisor immediately
Call fire services
Leave scene of incident if life appears to be in danger and gather at the muster Point at the sound
of the alarm for roll call
Do not go back to work until told to do so.
ASSAULT EMERGENCY
In case of assault by host community or attack by men of the underworld the following attitude shall be
displayed:

Do not retaliate any action from the attackers
Watch the number of persons involved in the attack.
Listen carefully and note their demands
Immediately after the attackers have gone carry out muster action to ensure that all persons on site
are safe.
Follow established procedure for MEDEVAC if any person is injured.
Obey as instructed by the attackers
BASIC EVACUATION PROCEDURES
All employees must study the evacuation plan, know the proper exits, corridors and responsibilities of their
section.
Each employee should know where to report upon evacuation, and should stay there until roll call is taken
or a supervisory authority gives further instructions. NO ONE IS ALLOWED TO RE-ENTER THE
BUILDING ONCE EVACUATED, UNLESS AUTHORIZED AND UNDER THE DIRECTION OF THE FIRE
DEPARTMENT.
At “no time” will anyone be allowed to stand in, or block any exits or corridors, nor will they be allowed to
remove any equipment such as files, etc. These areas must be kept clear for emergency personnel in
rescuing, firefighting or removal of dangerous substances. The appropriate agency (either Fire or Police)
will have the authority over employees in giving guidance for the direction of traffic flow for evacuation. The
removal of office equipment, appliances, files, etc., will only be removed by the Fire Department or with the
Fire Department’s granted authority to Company Officers.
Department Managers or Supervisors will take the roll call at the employees assigned assembly point. In
the event that a person is not accounted for, or there is some reason to suspect that an employee is in the
building, “immediate notification” should be made the Fire Department.
The removal of vehicles from the parking lot will be under the direct control and authority of the Fire
Department. (In some cases the Fire Department may request that vehicles be removed so that heavy
apparatus and equipment can be placed for the firefighting purposes if necessary. In other cases, due to
time and attack procedures, the Fire Department may request that no vehicles be removed so no
obstruction will interfere with the operations of the oncoming Fire Department vehicles with hose lines and
other emergency equipment).

ENVIRONMENTAL MANAGEMENT PLAN
The nature of our business is such that the by-products/wastes could pose a threat to flora and fauna (plant
and animals). In view of this however, we shall lay much emphasis on environmental concerns. As such we
shall ensure the following considerations while working on a project.
Carefully estimate the environmental hazards associated with our daily activities and make every
necessary effort to minimize the negative environmental impact of such activities.
Set up monitoring and assessment machinery.
Carry out environmental pollution and protection audits seeking for remedial action where
necessary.
Some adverse environmental effects could be cause from used oil, which has formed sludge,
mishandling of waste materials and scrap items. Proper and adequate care shall be taken to avoid
the careless discharge of by-products into the environment. As such we shall ensure that disposal
of waste are done by conventional methods, and that all work sites are reinstated to their original or
better form before our final demobilizing from the project site.
Toxic substances will be stored in such a way as to prevent accidental discharge into the
environmental.
In work areas where there are flammable gases, specific tests will be carried out and specific
instruction will be obtained from the client before take- off of the project.
Movement of people will be restricted from areas designated as hazardous and unsafe.
Store materials very safely.
Incident report must be prompt and accurate.
ENVIRONMENTAL FACTORS
These fall into three main categories in accident causation.
Physical Category: Noise, vibration, radiation, illumination and temperature extremes.
If operations on a machine produce high noise levels that can damage the workers' hearing, his
communications with others may be impaired. Thus, the workers may be unable to warn one
another of a hazard in time to avoid an accident.
Chemical Category: Toxic fumes, vapours, mists, smokes and dust. In addition to causing illness,
these elements often impair a worker's skill, reactions, judgment or concentration.
A worker who has been exposed to the narcotic effect of some solvent vapours may experience an
alteration of his judgment and move his hands too close to the cutting blade or unguarded part of a
machine.
Biological Category: Biological factors are those which are capable of making a person ill from
contact with bacteria, viruses and other micro-organisms or from contact with fungi or parasites.
WASTE MANAGEMENT
Waste management is a trivial issue; however, many times it is a contributing factor to occupational injuries
and illness. Due to our type and location of operation i.e. Civil Engineering construction, we gather our
waste into approved containers and at designated areas. Which in turn are disposed off by the refuse
collection contractors who will now sort them and they will be taken to approved dumping site.
That notwithstanding, we are concerned not only with the health and safety of those on our premises but
also with those away from our premises, who could be affected by our operations. It is important that any
waste, which we generate, is disposed off correctly so it does not create an unacceptable risk to others and
the environment. The responsibilities of our work force with respect to managing waste as started in our
policy are:

CHAPTER 8
OCCUPATIONAL HEALTH
The management will ensure that there is/are retainer-ship clinic available for use by personnel. The names
and addresses of the clinic be made known to all staffers. All staff of the company shall be medically
certified by the acknowledgement retainer-ship clinic. The certificates of fitness duly signed and stamped by
the clinic shall be documented in the office and sites of work. For very major projects, INDEXPRO INT’L
SERVICES LIMITED will establish site clinics, fully equipped and with site nurse. But for all projects, there
should be on site, fully equipped first aid box available and first Aider.
HEALTH AND IT’S OBJECTIVES
The primary objective of this HSE Plan is the safe commencement and execution of this project without any
accident. This can be achieved in the following ways:
Have an accident-free operation during our working, travelling and domestic life.
Reduce the rate of lost time injury (LTI), if any, to its barest minimum.
support the campaign to keep the environment clean
Increase the HSE awareness of the company.
Incorporate HSE in our monthly, weekly and daily site meetings, as an Agenda for discussion
Enforce the constant use of appropriate 'Personal Protective Equipment (PPE) and the right tools
for the job.
Ensure that permit to work is enforced at the work-sites
Provide adequate First-Aid Boxes and trained First Aider in the work-sites.
FIRST AID BOX/ACCESSORIES
A first aid box will also be provided on site for use by the first Aider/site nurse in administering first aid
treatment when the need arises. The content of the first aid box shall include the following in accordance
with client's standards:
1. Paracetamol Tabs (Used for the relief of body pain. 2 tabs for 3 times daily.)
2. Oral rehydration (Used to replace lost body fluid. Mix with a sachet with 2 mineral bottles filled with
clean water.)
3. Crepe Bandage (To keep dressing and splint in position.)
4. Cotton Bandage (To keep dressing in position.)
5. Triangular Bandage (Used as a sling to support part of the body.)
6. Cotton Wool Ball (Used for cleaning of wounds.)
7. Gauze Swabs (Used for wound covering.)
8. Hibitane lotion (Used for wound cleaning.)
9. T.B.C Lotion (Used to stain superficial abrasion.)
10. Handy Plaster (To cover minor wounds.)
11. Methy sal (Ben Gay) Topical application for the relief of muscular aches and pain.
At the commencement of the project, sufficient quantities shall be provided, but as work progresses, stock
will be replenished based on the advice of the site nurse. In order to monitor dispensing of drugs and
treatment, a first aid register will be provided for recording all cases.
House Keeping on Job Sites
It is necessary to keep a clean and tidy work environment for the comfort and convenience of the workers
as they arise in the work site and convey wastes so gathered to the clients approved dump site or
borrow pit for final disposal.
a.
Good housekeeping shall be maintained in stores, yards, building, vehicles, and job sites.
Supervisors shall be responsible for proper housekeeping in or around the work they are
supervising.
b.
Walks, aisles, stairways, fire escapes, and other passageways shall be kept clear of obstructions
and tripping hazards, fire extinguishers, etc shall be kept clear of obstructions.
c.
Tools and materials shall not be placed where they fall and strike anyone.
d.
Nails in boards, such as those removed from sheathing, scaffolds, forms, and packing boxes, shall
be removed and boards carefully stacked or stored, if they are to be reused. If such boards are to
be added to a scrap pile for burning, nails should be bent over or removed.
e.
Scrap materials of salvage value shall be promptly stored until suitable arrangements are made.
f.
Dirty and oily waste rags, trash, and other waste materials shall be deposited in approved
containers and disposed of as soon as practicable.
g.
Employees shall co-operate in keeping rest rooms, drinking cups, locker rooms, and other facilities
in a neat and sanitary condition.

LOST TIME INJURY: (LTI)
Work related injury which rendered the injured unable to perform regular job or restricted work on any day
after the day on which the injury occurred.
RESTRICTED WORK CASE (RWC)
A work related injury which result in a work assignment after the day on which the accident occurred, does
not include all duties of the person’s regular job.
FIRST AID CASE (FAC)
Any one time treatment and subsequent observation of minor scratches, cuts, burns, splinters and so forth
which do not ordinarily require medical cure.
NEARMISS ACCIDENT
A near miss accident is an accident which does not result in any injury or damage.

ACCIDENT, INCIDENT AND NEARMISS REPORTING
PROCEDURE
All accidents including Near misses, which occur in course of this job, will be promptly reported to Project
Engineer. The company’s Safety Officer has the responsibility of relaying all accident details to our
Managing Director and client’s representative.
Our representative will carry out a thorough investigation of the accident to determine the underlying
causes and advice on the necessary action to prevent future reoccurrence.
Such investigation report will be submitted immediately to the Project Engineer.
In accident cases involving a Lost Time Injury (LTI),and fatal accident, the Managing Director will lead the
investigation team.
ACCIDENT INVESTIGATION
Introduction
One of the essential features of effective safety management is the thorough investigation and follows up of
accidents.
The main objective of accident investigation is to prevent recurrence of similar accidents by identifying
deficiencies and recommending remedial actions. Follow up should ensure that those actions are
implemented.
Most accidents have more than one cause. Studies have shown that the accidents can have 20 factors
and underlying causes often exist away from the site of the accident. Proper identification of such causes
requires timely and methodical investigation, going beyond the immediate evidence and looking for deep
rooted conditions which may form the basis for future accidents. The right approach to any accident or
near miss is that it could be an indication of deficiency or failure in the management of the operation.
Every accident should be investigated, although the level of investigation and detail of the investigation will
vary and will depend on the actual or potential injury, damage or loss.
Management must support and be involved in investigations and be prepared to act on resulting
recommendations. Commitment is best demonstrated when a thorough and objective investigation is
carried out by capable investigators and prompt action is taken to correct deficiencies.
The Investigation Process
The accident investigation process comprises the following consecutive stages:

Analysis of Investigation findings, Positive Recommendations and Follow up.
1.
Accident Report
An accident report form should be filled within 24 hours of an accident. Even though the personnel
involved in the accident may not be available to assist in the making of an accident report, it is
important to make a preliminary report as soon as possible:
Advise operations so that alerts can be sent immediately to all other locations.
The accident report should be factual, and avoid hearsay, assumptions, and preliminary
conclusions.
The accident report must be signed by the site manager before it is forwarded to higher levels of
management.

Generally, it is best to limit the attendance to those directly involved or those who can give positive input.
Too many people from too high a level will restrict the flow of information.

Timing
Investigations should be carried out as soon as possible after the accident. The quality of evidence can
deteriorate rapidly with time, and delayed investigations are usually not as conclusive as those performed
with dispatch. A prompt investigation is a good demonstration of Management concern for safety.
c.
Scope
The scope of the Investigation can be divided into four areas:
Personnel
Technique
Third Party/Environment
Organization
In each of these areas, conditions, actions or omissions may be identified which could be factors
contributing to the accident or to subsequent injury, damage or loss.
3.
Establish the Facts
The objective of the investigation is to collect as many facts as possible which may help understanding of
the accident and the events surrounding it.
The initial stages of every investigation should gather and record all the facts which may be of interest in
determining causes.
At this point it must be emphasized that the purpose of the Inquiry Boards is NOT TO ESTABLISH BLAME
or decide PUNISHMENT. It is to determine corrective ACTION.
A trap to avoid in any investigation is that of reaching conclusions too early, and consequently failing to
keep an open mind and to consider the full range of possibilities.
a.
Background Information
It may sometimes be appropriate to obtain background information before visiting the accident site. For
example:
General procedures for the type of operation involved;
Records of instructions/briefings given on the particular job being investigated;
Location plans;
Command structure and persons involved.
b.
How to gather facts
In investigating an accident a systematic approach has to be taken to establish all the relevant facts about
how the accident occurred. In order to complete a thorough investigation the following approach should be
taken:
Inspection of the accident location
Interviewing of all personnel involved
Establishment of underlying causes.
4.
Inspection of the Accident Location
Important evidence can be gained from observations made at the scene of the accident, particularly if
equipment remains as it was at the time of the accident. Similarly, witness' statements can usually be
better understood and verified if discussed at site. It is not possible to set rules on 'freezing' equipment
and/or keeping people at a location, but as far as possible the site should be kept 'as is' until at least a
preliminary investigation has taken place. This is not always feasible, as rescue operations or the presence
of residual hazards and/or congestion may justify moving some of the equipment.
Local legislation may prescribe that for certain classes of accident, e.g. fatality or motor vehicle accident;
nothing may be moved without prior permission from nominated persons.
Photographs and/or video film will assist investigation and may have use later for training purposes.
Sketches can be useful in the absence of film or photographs.
The investigators should be looking for any conditions in the immediate environment which could have
contributed to the accident. Items to check include:
Position of sun/time of day
Effects of weather, wind, rain, dust clouds
Road topography
Road verge, condition
Position of all equipment at site of accident
Condition of equipment facilities
Illumination of location/site
State of housekeeping
Presence/absence of warning notices
Presence of witnesses, unauthorized people
Evidence of spills/release, colours/discoloration
Use of excessive forces.
a.
Interviews
Interviews should be carried out individually so that witnesses are not influenced by each other. Witnesses
should be asked to go step-by-step through the events surrounding the accident, describing both their own
actions and the actions of others.
The value of a witness's statement can be greatly influenced by the style of the style of the interviewer,
whose main task is to listen to the witness' story and not to influence him/her by making comments or
asking leading questions. This requires patience and understanding. If the investigation is a team effort,
great care should be taken not to make a witness feel intimidated by too many interviewers.
It should be remembered that an investigation team is often seen in a prosecuting role, and there may be a
reluctance to talk freely if people think they may incriminate themselves or their colleagues. An investigator
is not in a position to give immunity in return for evidence, but must try to convince interviewees of the
purpose of the investigation and the need for frankness.
At the end of an interview, the discussion should be summarized to make sure that no misunderstandings
exist. Any anomalies in the statement or conflict with other evidence should be discussed, the interviewee
being invited to clarify points as necessary.
b.
Conflicting Evidence
It is not unusual for different witnesses to have different account of the accident. Human memory is
notoriously erratic, and even if not motivated by self-protection or other subjective arguments, one person's
recollection of an accident can differ from another's in quite important details. Investigators should note any
significant differences in accounts of an event. Faced with conflicting witnesses' statements, investigators
should look for the similarities between the statements and commonalty with other evidence.
CHAPTER 10
HSE AUDIT, REVIEW AND INSPECTION
HSE Management shall form an integral part of overall site operation and made alive responsibility.
Supervisors who directly assign duties to workers must advise them on safety precautions compliance as
the job progresses. The Safety Officer shall carry out general HSE audit inspection weekly. Company
management team shall carry out monthly HSE audit on site. There shall also be technical safety audit on
equipment done by trained professionals. All the observations made during the audit shall be documented
and communicated through the line and shall be followed up. Reports on the various audits shall form
integral part of the agenda in safety meeting and tool box talk.
The following committees will be established and recognized by management and workers alike.
(a)
The Advisory Committee - will operate on a trip-partite basis and shall operate at a higher level of
safety policy making. They shall comprise of representatives from the inspectorates, trade union
and personnel department.
(b)
The Consultative Committee - this shall be responsible for specific health, safety and environmental
problem. It shall be their duty to gather the available expertise together, consult that expertise and
then decide on a course of action. The onus though is always on the individual to take some
form of action to resolve a problem.
(c)
The Recommendations Committee - this has the authority to discuss problems and to recommend
to management appropriate course of action. It may not have authority to see that its
recommendations are in fact adopted.
(d)
The Executive Committee - this shall be chaired by a Chief Executive
with the authority to
ensure that decisions reached by the committee are implemented. It may have a budget to finance
its day to day business.

WORK METHOD/CRITICAL ACTIVITY
INTRODUCTION
Accident don’t just occur, they are caused. Statistics has shown that most accidents occurred because the
proper working procedures were not adopted. All INDEXPRO INT’L SERVICES LIMITED staff is aware of
this trend. We have taken time to analyze the scope of work for and to develop work procedures aimed at
successful completion of the job.
The work procedures are segmented as follows.
Pre-mobilisation inspection

Permit to work (PTW) excavation permit system

Miscellaneous work / alterations

PRE-MOBILISATION INSPECTION
Pro to sending our staff, materials, plant and equipment to site, a team of engineers from OUR CLIENTS
will carry out inspection on all items to be used to ensure that they are in good working order, safe property
certified and insured.
This inspection forms part of the mobilization which can only be certified complete with all necessary
personnel and equipment, materials and site facilities are present on site and work is ready to commence.

proper execution of projects will be available when required
MOBILISATION
This shall involved the mobilization to site all necessary personnel, equipment and materials and generally
made ready to commence work
PROVISION OF HSE REQUIREMENT
The management of HSE related issues has been an integral part. INDEXPRO INT’L SERVICES LIMITED
business and as such all HSE requirement will be provided prior to the commencement of the project
bearing in mind that all hazards have been systematically identified and the full implication of all their
potential effects known and addressed. This includes all implications of hazards on all person affected by
operations, this impact on the environment and host communities.
PERMIT TOWORK (PTW) EXCAVATION PERMIT SYSTEM
Prior to the project kick off, permit to work form and excavation permit will be obtained from Client
representative on site. The permit to work form will be validated on daily bases. The condition of the permit
to work / excavation permit will be complied with during the excavation of the job
SITE CLEARING
This includes clearing of the bush and grasses around the work environment and the removal of all
unwanted material from site.
ANTICIPATED HAZARDS
- Injury due to improper handling of tools
- Limitation due to contract between the skin and grasses
- Littering of sharp object around working area.
PRECAUTIONS

TRAINING
All people handling asbestos should be adequately informed of the hazard and should be fully trained in
hygiene and safe work practices.
DEMOBILISATION
This will involve the removal of the company’s equipment, tools and unused materials from site. This will be
followed by thorough clean-up of the site, then handed over to Client.
ELECTRICAL SAFETY
The purpose of this plan is to protect employees working on or near sources of electrical hazards,
conductors and parts of electrical equipment that have been de-energized but have not been locked out or
tagged in accordance with these procedures shall be treated as energized parts.
GENERAL ELECTRICAL REQUIREMENTS
Only authorized personnel shall make changes or adjustments in electrical equipment. If you do not have
the proper knowledge, leave electrical repairs to experienced personnel.

Approved fuse testers and “pullers” shall be used. The proper size fuse or breaker for each circuit
shall be marked in the fuse/breaker specification.

Protective lineman’s rubber gloves and leather gauntlets shall be available for protection when
working with electrical circuits. Protective electrical gloves shall be inspected prior to use and tested
annually.

Except for double-insulated electrical tools, all tools must be equipped with three-pronged grounded
cords and plugs. The plug-grounding prong shall not be cut off, bent, or otherwise modified. If it is
necessary to use a three-prong plug in a two-prong receptacle, ensure that the tool is properly
grounded.

Electrical power tools and cords shall be inspected before being used.

Defective equipment must not be used!

Fixed electrical machinery (e.g., lathes, grinders, drill presses, etc.) shall be installed in accordance
with applicable section of the National Electrical Code.

When working near live switch-boards or contacts, don’t let tools stick out of your pocket or tool belt
and strike live contacts.

Do not use portable electric equipment if your hands are wet or if you are standing on wet ground.
All INDEXPRO INT’L SERVICES LIMITED “Gang” boxes are equipped with ground fault receptacles.
Ground Fault Receptacles will be used for all power tools on all INDEXPRO INT’L SERVICES
LIMITED job sites.
ELECTRICAL CORDS
Keep a constant check on electrical cords, paying attention to their condition and their placement.

Do not splice cords if they have been damaged; replace them

Use the correct size cord for the job at hand.

Do not carry tools by their cord.

Except for double-insulated tools, cords should be of the three-wire type (with three prong
grounding) plugs.

Disconnect equipment by removing the plug from the receptacle.
Never disconnect by pulling on the cord itself.

Adapter cords (cheater cords) shall be provided with an explosion proof plug on one end and a
three-wire grounding type receptacle on the other end.
WRITTEN PROCEDURES
INDEXPRO INT’L SERVICES LIMITED shall maintain the written copy of procedures and make them
available for inspection by employees, client, or regulatory inspectors.
APPLICATION OF LOCKS AND TAGS
A lock and tag shall be placed on each disconnecting means used to de-energize circuits and equipment
on which the work is to be performed.
Each tag shall contain a statement prohibiting unauthorized operation of the disconnecting means and the
removal of the tag.
If a lock cannot be applied, INDEXPRO INT’L SERVICES LIMITED personnel must demonstrate that
tagging procedures will provide a level of safety equivalent to that obtained by the use of a lock, then and
only then a tag may be used without a lock.
A tag used without a lock, shall be supplemented by at least one additional safety measure that provides a
level of safety, equivalent to that obtained by the use of a lock. Examples of additional safety measures
include the removal of an isolating circuit element, blocking of a control switch, or the opening of an extra
disconnecting device. A lock may be placed without a tag only under the following conditions.

Due to the nature of our job and its location in the work site, it is imperative to note that security has already
been provided. But Landowners will ensure security of lives and property during its operation.
SECURITY EMERGENCY RESPONSE PLAN
INTRODUCTION
The initial and short-term response to a serious security incident, which can turn out to be an emergency, is
very critical to the final outcome. Managers of emergencies therefore need to make proper evaluation and
verification of the threat and make an assessment of likely effect.
This emergency response
plan/procedure is aimed at creating an organized and structured response strategy to very serious security
incidents that may occur at Production Facilities or affect its operations. The essential purpose is to
remove threat to life, safeguard property and information.
DEFINITION OF TERMS
A security Breach or Violation is an act or omission committed by a person or group of persons, which
contravenes the law. Minimal effect to business could be treated routinely by security personnel in
accordance procedures.
An EMERGENCY is an unexpected occurrence requiring an immediate action. It could be an incident or
state of danger having or like to have, a potential or immediate serious effect on Company staff, property,
business activity, e.g. violent community demonstration/disturbances, serious attacks on satellite
accommodation, etc. and emergency requires control by senior management in accordance with
contingency procedures.
A CRISIS is an event or sequence of events of extreme difficulty, distress or danger, which the Company is
unable to contain within is own control and resources.
In an emergency, all appropriate emergency measures should be implemented and assistance given to the
authorities in order to minimize any potential or actual loss of life and/or property, threats to the continuity or
business and any adverse effect on the corporate image.

Before engaging in work in public right-of-way, warning signs or traffic control devices shall be
placed conspicuously, to approaching traffic. Where further protection is needed, suitable
barricades shall be erected. Where the nature of work and traffic requires it, a flagman shall be
stationed to warn traffic while the hazard exists. The designed flagman shall wear a safety vest.

In case openings or obstructions in the street, sidewalk, walkways, or on private property are being
worked on or left unattended during the day, danger signals such as warning signs and flags, shall
be effectively displayed.
