DESCRIPTION OF INCIDENT Mr. Bost Jalibe said at exactly 9:30 pm., Telma Laude was about to go to the ground floor, she came from the second floor. Telma just went to her house, but she forgot her phone, so she came back to the office because her phone was left in her drawer on her desk. Mr. Jalibe said that Telma didn’t even go through the right side stairs when she was going down to the ground floor, she went through the left side stairs that got hit by the rain then the stairs got wet and Telma didn’t noticed that there was water on the stairs from the second floor to the ground floor, so she quickly went down, and when she went down, she slid about 5 steps from the landing to the ground floor, so she slid and rolled and she broke her right hand above the elbow on her right hand, and we were unable to help her because she didn’t scream right away to ask for help because she couldn’t talk and scream because of the pain of her broken hand. About 10 minutes later, I saw Mr. Bruno Mars and we heard her shouting and said “Help me, help me” and we just knew that Telma was the one who screamed and asked for help, so we called an ambulance so that we could take her to Holy Name University Medical Center because her hand was broken. So that’s what we did for Telma, maybe even 5 minutes after we called an ambulance, it arrived immediately. Their response was very fast. We also contacted Telma’s husband and her mom and dad so they could know about the accident that happened. After, we called the head of our company so we could inform her immediately about the accident so that she could help Telma with the insurance. It’s because we have insurance for each of employees here, it’s worth 100,000 pesos especially if there is an accident. Because Telma is a regular employee, she worked here for 8 months in our company, so Telma is covered by insurance and she doesn’t have to pay anything. Mr. Jalibe hopes that Telma’s hand is going to be okay. ACTION TAKEN AFTER THE INCIDENT The maintenance team was advised to keep an eye on the business premises during business hours, especially in the event of unfavorable weather. Additionally, to avoid a repeat of the previous disaster in the inside of the company. Caution wet Floor and other pertinent safety signage were placed by the maintenance personnel.
