As the cyst fills or becomes infected, it can become tender. An incision and drainage can be performed, but the cyst must be surgically removed. Resection is needed possibly due to Necrotizing Enterocolitis (NEC) or Gastroschisis. Pt requires nutritional support. Liver Problems Cirrhosis Complications: Portal hypertension, ascites, esophageal varices, coagulation defects, jaundice, portal-systemic encephalopathy (& coma), hepato-renal syndrome, spontaneous bacterial peritonitis. Hepatitis Cancer of the Liver Liver Transplant Biliary & Pancreas Problems Cholecystitis Cholelithiasis Pancreatitis Acute Chronic Pancreatic Cancer Other (not in text): Biliary Atresia: Obstruction of bile flow due to absence of some or all biliary ducts. Etiology is unknown. Therapeutic mgmt.: exploratory laparotomy and Kasai procedure (hepatoportoenterostomy) to connect the bowel lumen to the bile duct remnants. Most successful in infants up to 10 weeks. Liver transplantation may be needed if not found early enough. Nursing Assessment and management: chalky/white stool, jaundice, hepatomegaly, splenomegaly. Labs (elevated bilirubin), ultrasound, biliary scan, liver biopsy. Nursing care is focused on vitamin and caloric support (fat soluble vitamins A D E & K).
