**Agency** is pleased to make a commitment to work with Metro Community Ministries Inc.’s
subgrantees and affiliates as they implement Urban Leaders program, serving youth and young
adults across the country. **Agency** has facilities and sites in each of the target regions, and
makes a commitment to provide ongoing support to the program and its participants through
opportunities with our corporation.
**Agency is justice system partner (identify which justice system-police, sheriff, DA, community
corrections, court, public defender). As a commitment to the Urban Leaders’ program,
*Agency* will:





Assist with referrals of high risk individuals to the program;
Share documents or data, which may include risk assessments, with participant consent
as appropriate;
Collaborate on multi-disciplinary team meetings
Sit on the Urban Leaders Advisory board
Support Metro Community Ministries Inc.’s affiliates in its efforts to serve the target
population.
