AFFIDAVIT OF LOSS
I, LERIO BRENDIA PASAL, also of legal age, married, Filipino and a
resident of San Antonio, Impasug-ong, Bukidnon Philippines, after having been duly
sworn to in accordance with the law, hereby depose and state THAT:
1. I am the Sister of Mary Ann Guingao;
2. Mary Ann Guingao is the spouse of Jose Mendez Guingao;
3. Jose Mendez Guingao is the registered owner of a parcel of land located at Brgy.
Binauganan, Tarlac City which is covered and described under Transfer
Certificate of Title No. 386707;
4. On _____, Jose Mendez Guingao passed away thereby leaving all his properties
to my sister Mary Ann Guingao and their son Jos Angel B. Guingao as his sole
compulsory heirs;
5. Sometime on ____(After adverse date)_______, Mary Ann Guingao delivered
the original title owner’s duplicate copy for safekeeping before she left for the
United States of America;
6. I then kept the original owner’s duplicate copy of the said title in a cabinet
inside our house where I also keep other important documents of mine;
7. Recently, my sister called me and asked about the original owner’s duplicate
copy of the said title. It was then that I found out that it is missing;
8. I exerted diligent efforts to locate and recover the same, extending my search
to the premises of our house, but to no avail;
9. I am therefore treating the original owner’s duplicate copy of Transfer Certificate
of Title No. _____as lost for all legal intents and purposes;
10. I am executing this affidavit to attest to the truth of the foregoing statements
and for all legal intents and purposes it may well serve.
