My name is Yazmeilis Santos and I am a university student at Universidad
Interamericana de Arecibo completing my practice at Reborn Christian Academy. You
are receiving this letter to inform you that as a requirement for my graduation, I have
to record myself giving
classes, and the students in the class will be present in the
video as well. I understand if you are uncomfortable with your student being on
camera, so this letter is for consent to have your students recorded for the
classes as
previously stated. These videos sole purpose is for my grade, and will not be used for
anything else. I appreciate your cooperation in advance. Please return this letter to the
school via your child’s backpack so that we can keep track of these consent forms. Feel
