Click the hyperlink above or at the end of the guidelines to read the case study. 1. Why do you believe the different elements of CareSouth Carolina have allowed it to have a positive impact on the communities it serves? 1. How might its three-tier approach be part of this impact? 2. What elements of CareSouth Carolina would work in rural Georgia clinics and why? (Be specific) 3. Based on the information from class and the case study, pick one of the Mercer Medicine clinics located in a rural county (all but the downtown Macon clinic) https://www.mercermedicine.com/locations (https://www.mercermedicine.com/locations) , and provide specific information on how you would replicate or reinvent some or all of what CareSouth Carolina does within the clinic. Keep in mind that health statistics and demographics may be different so services provided at the clinic need to reflect this difference. 1. You need to provide data on the county the clinic is located and surrounding rural counties (2 to 3). (You can find this information using county health rankings or OASIS) 1. Number of residents, health outcome information, other information you feel is relevant 2. After you provide data on your county, you need to explain specifically how you would replicate or reinvent elements of CareSouth Carolina in that clinic location. 4. Explain (in detail) how providing these services within the clinic could impact obesity, mental health, health care delivery, food security, and aging concerns within the county the clinic is located and the surrounding counties that may seek out the clinic for services.
