Avoid accidents, clean up immediately Wear protective equipment to prevent skin and eye contamination and the inhalation of vapors Work up wind or increase ventilation Contain spill, prevent run off into drains and waterways Use absorbent (soil, sand or other inert material) Collect and seal in properly labelled containers or drums for disposal Fire Hazards Storage tanks and dispense pumps are adequately maintained and monitored Permit Issuer, Permit Receiver, Construction Supervisor, HSE Officer, Driver, Worker JOB SAFETY ANALYSIS 3. Positioning of refueling truck and storage tank/equipment in position Fire Hazards Collision Hazards Filling / Refueling Spillage Do not refuel while near any open flames Strictly prohibited to use mobile whilst refilling/ refueling Do not overfill the fuel tank (95% full is a good guideline for any type of vehicle or container) On hot days, allow room in the tank for fuel to expand. Place fuel containers on the ground when refueling, never on the bed of the work vehicle and keep the nozzle in contact with the fuel tank’s inlet tube to dissipate the build-up of static electricity that could cause a spark Grounding / bonding must be in place during refueling to prevent spark generation Use only approved metallic containers for temporary storage purpose Ensure appropriate type Fire extinguisher is available with inspection tags Follow the instruction given on safety signage Reduce the vehicle speed at the speed bumps on the road No Overtaking is allowed in the Site premises Driver shall have valid SA driving license and he should be engaged after getting proper HSE Induction, defensive driving training Strictly prohibited to use mobile whilst driving Drivers/operators should be provided with Emergency contact numbers Make sure vehicles lights are functional all the time especially at night Ensure First aid kit is always in the vehicle Ensure Fire extinguisher in vehicles with appropriate inspection tags Seat belt shall be used by both driver and passengers. The fuel cap and nozzle are mounted without tripping or falling 4. Appropriate stock management methods are followed Identify hazardous areas and control all sources of ignition - use appropriate warning and hazard signs Ensure appropriate type Fire extinguisher is available with inspection tags Place fuel containers on the ground when refueling, never on the bed of the work vehicle and keep the nozzle in contact with the fuel tank’s inlet tube to dissipate the build-up of static electricity that could cause a spark Use only approved metallic type containers for temporary storage purpose Permit Issuer, Permit Receiver, Construction Supervisor, HSE Officer, Driver, Worker Permit Issuer, Permit JOB SAFETY ANALYSIS Fire Hazards Health effects when handled by individuals e.g. Dermatitis hazards Remove fuel cap on machine and wait for any air pressure to vent from the tank Insert the fuel nozzle into the fuel tank ensuring that the nozzle is fully inserted into the fuel port Pump fuel at allowable rate always remaining in position with one hand on the nozzle When tank is full release nozzle and position the nozzle in upright condition Delivery hoses will be turned off and locked when not in use. Hoses and valves will be inspected regularly Drip tray is to be placed under discharge nozzle Refueling line should be kept as short as possible To stop pumping before any fuel overflow The filling/refueling process will always be attended by competent person Required stocks of spill absorbents shall be maintained Appropriate Fuel pump must be used for dispensing fuel Water pump should not be used for dispensing fuel The equipment’s/vehicle shall be turned off prior to commence refilling/refueling Do not refuel while near any open flames Strictly prohibited to use mobile whilst refilling/refueling Do not overfill the fuel tank (95% full is a good guideline for any type of vehicle or container) On hot days, allow room in the tank for fuel to expand Place fuel containers on the ground when refueling, never on the bed of the work vehicle and keep the nozzle in contact with the fuel tank’s inlet tube to dissipate the build-up of static electricity that could cause a spark Grounding / bonding must be in place during refueling to prevent spark generation Use only approved metallic containers for temporary storage purpose Ensure appropriate type Fire extinguisher is available with inspection tags Filling & refueling personnel must wear FRC coverall & Face shield while filling & refueling Avoid eye and skin contact whilst filling/refueling and wash hands after Receiver, Construction Supervisor, HSE Officer, Driver, Worker JOB SAFETY ANALYSIS Electrical Hazards Appropriate type PPE and RPE are provided All electrical equipment used out of doors should be suitably insulated and should be supplied through a Residual Current Device All electrical switchgear controlling machinery should always be clearly labelled and readily accessible. The fuel dispensing truck and storage tank/equipment’s shall be grounded. Nobody shall not reenter the equipment/vehicle while refueling or if necessary, shall make contact with the body of the equipment/vehicle, which is grounded; for discharging the electrostatic charges. Appropriate fuel pump must be used for dispensing fuel. Water pump should not be used for dispensing fuel Filling and Refueling personnel must wear FRC coverall & face shield while filling & Refueling Spillage Fire Hazard 5. Disconnecting and clean up Spills – Environmental damage Explosions Let the fuel nozzle drain for a few seconds before removing Remove nozzle slowly avoiding any spills or drips Wipe any residue fuel from tank port Position the nozzle in upright condition Replace cap on fuel port ensuring it is secure Check for leaks Lock up / secure any fuel pump units 6. Manual handling Incompetent personnel Pinch point Personal injury, back injuries, muscle strains, sprains. Slip, trip fall on same levels Ensure trained and competent workers who will carry out the task Pre-employment checks of competence Keep away hand & finger from any pinch point hazard Wear approved hand gloves Eliminate all unnecessary manual handling Train staff in proper lifting techniques Provide suitable equipment e.g. keys for lifting manhole covers Avoid lifting items which are too heavy - use a trolley or castors where possible Housekeeping shall be a priority, walk and work areas shall be kept free of materials and supplies Remove unnecessary materials in the workplace Personnel to be trained in oil spill cleanup and safe disposal Spill kit must be available on site Eyewash should be available Good housekeeping skills will be used to keep the area free of rubbish 7.
