STEP 1: Rename the file by adding your last name and first name
LASTNAME, FIRSTNAME

STEP 2: Unblock the file from the file
properties.
a. Select the file, right click.
b. In the General Tab, tick the
unblock checkbox
c. Click Appy and OK

STEP 5: The floating Menu will be displayed on the screen. This
can be dragged and position to any part of the Microsoft Excel
Window.

Personnel Profile
• Type Complete Name,
• Employee
• Choose Position, Age, Sex
Level Taught, Grade Level
Taught and Subject Taught
from the dropdown list.

• Choose Number of Years in Teaching, Highest Degree
Obtained and Area of Specialization from the dropdown
list.
• Type Major Obtained under the Highest Degree
Obtained if applicable

• Type complete name of your rater, his/her Position, Date
and of Review

• Type Region, Division, Municipality, School ID, School Name,
School Type, School Size and Curricular Classification

• Type Date Observed and Name of Rater

• Type Subjects taught during the
classroom observations
• Rate COT Indicators by referring from
the rating reflected on the COT Rating
Sheets by typing 3 to 7 (For Proficient
Teachers). You may also choose from
the dropdown list.
• For Proficient Teachers COT Indicators
6-9 are either Observable or NonObservable

Observable/Non-Observable MOVs
• For Proficient Teachers COT Indicators 6-9 are either Observable
or Non-Observable. Type or Choose NA if indicators from 6 to 9
are No Acceptable evidence was shown, this will enable the
Non-Observable MOVs (SLM, DLL, WHLP, Video Lesson, Audio
Lesson, Others). TRF must be presented and rated if indicators
8-9 were in NA status.

• For Proficient Teachers, Objective 2 requires Lesson Plan with annotations
• For Proficient Teachers, Objective 11-19 requires valid MOVs based on the
suggested MOVs per objective.

classroom observations
• Rate COT Indicators by referring from

Proficient Teachers). You may also

Indicators 6-7 are either Observable or
Non-Observable

Observable or Non-Observable. Type or Choose NA if indicators
from 9-10 are No Acceptable evidence was shown, this will
enable the Non-Observable MOVs for TRF ratings.

• For Highly Proficient Teachers, Objective 2 requires Activity Sheets with
annotations
• For Proficient Teachers, Objective 11-19 requires valid MOVs based on the
suggested MOVs per objective.

• Choose from the dropdown list for the Level of Capability and Priority of
Areas to be Address for each objective and indicators.

Self Assessment Tool (Part II A. Functional Objectives)
• Summary of Strengths and Priority Areas to be Addressed will be listed
automatically. This will serve as your bases in the preparation of Professional
Development Plan under Functional Objectives.

Self Assessment Tool (Part II-B. Behavioral Objectives)
• Tick the checkbox assigned for each indicators under Behavioral Objectives.
• Total and Description will be automatically assigned based on the following
scales:
Descriptive Rating: 5 - Role model | 4-Consistently demonstrates | 3 Most of the
time demonstrates | 2- Sometimes demonstrate | 1-Rarely demonstrates

• This sheet summarizes the Strengths and Priority Areas to be Addressed
under the Behavioral Competencies as bases in the preparation of

E. FINALIZING AND SUBMISSION FOR SCHOOL CONSOLIDATION
• SAVE AS DRAFT will save the data for reviewing
purposes
• PRINT THIS SHEET will allow to print the current
sheet.
EXPORT DATA will finalize and will generate Individual Database for submission and
school consolidation

NOTE: IF EXPORTING DATA WAS INTERRUPTED BY A SYNTAX ERROR, JUST IGNORE
THE ERROR.

REQUIRED SOFTCOPIES TO SUBMIT FOR SCHOOL CONSOLIDATION
• Copy of Accomplished Automated IPCRF. Filename must be in the following
format:
• Copy of the Exported Individual Database. Filename must be in the following
format: PT-Dbase_YourLastname or HPT-Dbase_YourLastname
Note: PT-Dbase syntax is for Proficient Teachers
HPT Dbase is for Highly Proficient Teachers
