AFFIDAVIT OF KINSHIP
(DIRECT DESCENDANT)
I, MELINDA ROMANO PERICO, of legal age, Filipino, married and a
resident of Purok 1, San Francisco, Cauayan, Isabela, under oath, hereby
states:
1.
That I am one of the surviving heirs of the late CANUTO
ROMANO and his deceased spouse ANASTACIA AGCAOILI;
2.
That during the subsistence of their marriage, they were blessed
with three (3) children: JUANITA ROMANO SABINO, JUANITO
ROMANO, and CARMELITA ROMANO VILLALUZ, who were now all
deceased;
3.
That I am the sole surviving heir of JUANITO ROMANO; hence,
a legal heir of CANUTO ROMANO, by right of representation, and a direct
descendant;
4.
That I executed this affidavit of heirship for the purpose of
requesting certified true copies of deaths of CANUTO ROMANO,
ANASTACIA AGCAOILI ROMANO, and JUANITO ROMANO, to be used
as attachments in an extrajudicial and/or judicial settlement of the estate of
the late Canuto Romano, located in Malaram, Allacapan, Cagayan;
5.
That I executed this affidavit in good faith to attest the
truthfulness of the foregoing and for whatever legal purposes it may serve.
