h. Disclosure
2. Company Profile – to include but not limited to :
a. List of clients and accomplished projects
b. Complete product list
c. Updated price list
d. Affiliates/partner firm
e. Certificate of accreditation with other suppliers/firms
f. Copy of served Purchase orders issued by other customers – major projects
g. Certificate from other customers
3. Business permit latest (to be updated every year)
4. SEC or DTI registration and Articles of Incorporation
5. BIR registration/Certificate of registration
6. Authority to Print
7. 2 years Audited Financial Statement (latest)
8. SSS and Dole Certification
9. PCAB License, if any
10. List of production machineries and delivery equipment
11. List of manpower
12. Affiliated financial institution
