Assignment #2: 1. The patient’s assessment behaviors are indicative of a neuro focused assessment. The patient is confused so it is likely that the patient is not AAOX3. The case also mentions that C.J. has strength deficits to her extremities which likely indicates a neurological problem. She also does not have facial symmetry because she has facial dropping on her left side. Another area in the physical assessment that should be explored is the integumentary focused assessment to check for any lesions. It is important to check her skin integrity because it is likely that she fell as she was found on the floor. 2. This patient scenario is indicative of a respiratory focused assessment because the patient has shortness of breath and sputum. The patient may have pneumonia because the patient’s sputum is yellow now. The patient could also have COPD. I would listen to his lungs and check the pulse oximetry. I believe that also doing a cardiac assessment would be a good idea. 3. I believe that this patient scenario is a cardiac focused assessment because the patient has chest pain, is tachycardiac, and has an elevated blood pressure. She also has an abnormal heart sound. I would check all her pulses and listen to the apical site for 1 whole minute. I would also include a pain assessment that involves PQRST to find out more information regarding her chest pain. My patient scenario: A 25-year-old male with a history of seven weeks complaint of heart burn that worsens with lying down after dinner. He has pain and difficulty swallowing. Upon assessment, the patient reports that he gets heartburn after eating burger and fries. He mentions that the heartburn keeps him awake at night. He mentioned that antacids help relieve symptoms. He has no known allergies.
