) agrees as follow:
1. I receive the
Brand Mobile Tablet provided by Fair Deal Company Limited in order to use in
Distribution Management System as per following list:
a.
Tablet
b. Power Charger with Cable
c. Tablet Cover Casing
d. Sim Card No.

2. As I am allowed to use it to conduct sales more efficiently and quickly, I will use Mobile Tablet respectfully.
3. Apart from pre‐installed optional applications settings at the time of delivery, modifying other settings,
installing or uninstalling applications are not allowed.
4. Leaving Seals and Stickers on the tablet at the time of delivery, uncovering Tablet or unscrewing Tablet are
not allowed
5. Replacing provided sim card by own consent, using provided sim card at own or other’s accessories are not
allowed
6. Transferring provided Tablet and accessories, sim card to transfer to other party by own consent, make use,
leave uncover in public place, showing to another person are not allowed.
7. Regarding power failure, power problem or other tablet concerned or sim card concern problem, repairing
fixing to submit to service center by own consent is not allowed. If any failure occurred, prompt inform to IT
Department and follow instructions of department.
8. Loss or damage of Mobile Tablet, Sim Card, accessories; not allowed to replace by own consent. If such case
occurred, firstly inform to supervisor and supervisor must inform to Admin Dept, HR Dept and IT Dept for such
event. IT Dept will check and if such damage or failure is due to user’s careless action, the user must pay such
replacing and repairing cost, fees as a whole or in part. In the event of a loss, the user must pay the total cost
of accessory as a whole.
9. When IT ask Tablet for configuring, upgrading provided Applications, Firmware or other security settings, must
submit to IT Dept.
10. Due to transfer other department, resign, dismissal or when company ask to handover, user must hand over
each and all accessories back to company.
I sign agreement to abide and follow above points with my free independent consent.
