CONSENT AND RELEASE
The undersigned hereby grants to COMPANY NAME licensees and assigns (together
called the "PERSONS"), its directors, employees, agents, representatives licensees and
assigns, perpetually and irrevocably, the unconditional right throughout the world to
record my likeness [and/or my voice] on any visual medium, to edit such recording at
Producer’s discretion, to incorporate the same into the televisions series “Kim’s
Convenience” in any and all media, throughout the world in perpetuity.
The undersigned hereby releases and discharges PERSONS, its respective directors,
employees, agents, representatives, successors, licensees and assigns from all claims,
demands, or causes of action that I may have for libel, defamation, invasion of privacy or
publicity, infringement of copyright or violation of any other rights arising out of or relating
to any use of the rights or based on any failure or omission to make use of such rights.
In full understanding of the rights granted and the release set out above, I have signed
