to pre-implementation stage. *
1 ) The school head conducts an orientation program on monitoring and evaluation and plan adjustment (MEPA)
mechanism to implementation.
2) The school head clearly defines roles and responsibilities, like the functions of MEPA teams/coordinator and
involvement of SIP/AIP project teams.
3) The school M&E team (SMET) consistently prepares for MEPA activities at the end of each quarter, following the theme
on the toolkit.
4) The school head explains to the SMET the tools/templates to be used in reporting before they begin collecting and
organizing data.
5) The school head convenes MEPA/project teams and gather data during the pre-work, individually or in small groups.
6) The school head leads the SMET on accomplishing the output matrices and computing physical accomplishments, vis-àvis financial accomplishments.
7) The program proponent/project team leader presents all project accomplishments to date and variances.
8) The program proponent/project team leader includes general observations in the working PMRF to provide input for
midyear/year-end project monitoring.
9) The SMET accomplishes the issues tracking board and discusses possible factors that may have affected implementation.
10) The designated documenter/rapporteur prepares a narrative report of the pre-work or minutes of the meeting.

1) The school head consolidates the quarterly physical and financial accomplishments submitted by MEPA teams/project team
leaders/program proponents, and schedules another session/plenary with the SMET.
2) The SMET validates the consolidated quantitative and qualitative information gathered during the pre-work to be presented
by the school head.
3) The school head and SMET carefully use dashboards and graphical to present education data on quality, access and
governance.
4) The SMET covers completely the school M&E content areas, such as delivery of basic education services and organizational
performance.
5) The SMET discusses the balance of physical and financial outputs and identified reasons for variance.
6) The SMET focuses their deliberation on the assessment of common bottlenecks/issues/gaps in the implementation of
programs, activities and projects (PAPs).
7) The SMET collaboratively proposes recommendations or interventions to address bottlenecks/issues/gaps and apply
problem-solving techniques.
8) The SMET identifies value-added contributions of program proponents/project teams/stakeholders.
9) The program proponents/project teams report their lessons learned in program/project implementation, and describe
measures to sustain notable strategies employed.
10) The designated documenter/rapporteur prepares a narrative report of the MEPA proper or minutes of the meeting.

1) The school head, together with the SMET, writes the activity completion report (ACR).
2) The SMET prints the finished ACR and furnishes the school head, program proponents, and project team leaders a copy for filing
or reference in the following quarter.
3) The program proponents/project team leaders make their respective plan adjustments for the succeeding quarter in reference
with their original action plans.
4) The program proponents/project team leaders organize their program/project MOVs for the quarter into a print or digital
portfolio.
5) The school head updates his/her technical assistance/instructional supervision plan based on key inputs and issues/gaps
determined during the MEPA proper.
6) The school head presents the salient MEPA data teaching/non-teaching personnel and stakeholders during quarterly
assembly/PTA conference, or SOSA.
7) The SMET displays a summarized/graphical ACR on the basic education information system board and/or any conspicuous area/s in
the school to inform school-goers of quarterly school data.
8) The school head submits the MEPA report to the SHIGO/PSDS as input for TA and/or district consolidation.
9) The SMET celebrates success, and recognizes deserving program proponents/project teams/stakeholders and major
contributions for school effectiveness.
10) The school head implements a feedback mechanism (online/offline) to further improve the MEPA processes, as well as
effectively and efficiently implement school PAPs, through comments/suggestions from school stakeholders.
