Name of official completing the form: the person who will sign and sealed the documents with his/her
ID number (Optional). He/she can print the name like as sample as KIRAN KUMARI or just type the name
as usual.
Title: it means what is the position of the authorized person like staff, Clerk, registrar officer or
whatever.
Telephone: must provide the genuine official number of the authorized person like University’s official
number of his/her…if they don’t have it give their general contact number.
Email: his/her university official email-ID …..@lpu.in
Date: when they will attested the documents and sealed the documents.

Moreover, please refer to the document submission checklist and ensure that
everything is fine by ticking the checklist.
The documents which must be inside the sealed and signed envelopes as follows:
1. Photocopy of Degree Certificate which I send you through the email. Don’t need attested or
signed and it should be black/white photocopy not the colored one.
2. Official Academic transcripts must be properly signed by official and stamp of university should
be there. Please see the first page I already mentioned it. (Officer knows which ones)
3. Academic request form and document checklist.
All these documents must be present in a single envelope and then sealed it and signed it.
Make sure it must be like this way and write the Toronto address with reference number must be there.
