Training Agreement This agreement is between Child Wellness Institute of New Jersey (ChildWIN) and Hamilton School District and confirms the details of the following commitment: Training Topic/Title: Resilience and Advanced Strategies 3 Promoting Safe Schools: De-escalation Strategies for School Staff (Part 1 and 2) Peer Reflection Date: October 1st - June 15th, 2021 - See attached schedule Time: (start/end) Highschool- 2:45-3:45 Middle School- 3:15-4:15 Elementary School- 3:45-4:45 Location: Virtual – Zoom Links provided by ChildWIN Audience: HSD staff (Elementary, Middle, and High schools) Maximum amount of participants 99 ChildWIN agrees to provide: A fully-trained Instructor/Facilitator Training materials for each participant, including pre/post tests and course evaluations Training certificates of attendance for each individual who attends the full training, if requested Provide any documentation, upon request, necessary for Organization to apply for continuing education units Organization agrees to: Pay ChildWIN $40,300.00 Provide audio/visual equipment if needed for training participants Identify participants for course registration and enrollment Cancellation Notice: In the event of a cancellation, a notice must be submitted in writing and received by the PCA-NJ offices at least 5 working days prior to the scheduled event. If the cancellation occurs fewer than 5 days prior to the scheduled event, the Organization will be subject the total fees as previously agreed upon. This agreement is accepted in whole when signed by Agency and ChildWIN representatives.
