INFORMED CONSENT FORM
Please read this consent agreement carefully
Literacy Training Service is geared towards activities that have social impact through
activities that could contribute to health, education, environment, entrepreneurship, safety,
recreation and morals of the citizenry. NSTP-LTS2 stressed the importance of youth involvement
in broad programs or activities that will benefit the people through the basics of reading, writing,
and numbers.
Objectives:
1. To develop a sense of volunteerism in serving others by reaching out and engaging in
a free service to the people in our community.
2. To share and extend the skills and knowledge in the community especially in education
sector.
3. To prepare and train students to work effectively in the new normal set-up.
What will the beneficiaries do in the study: The learners from the partner school will be the
primary recipient of data allowance that they may use for attending the tutorial session program
given by the LTS students. They may also access the materials used in every session through
Facebook Group Page exclusive for the partner school.
Participation: The participation in the study is entirely voluntary, the faculty-in-charge will
identify learners who are in need of the Tutorial Service.
Payment: The LTS students will not receive any payment for this Tutorial Service Program since
this will be part of their voluntary partnership and requirement in the course.

There will be a minimum contribution amounting to Php 150.00 to shoulder
the load allowance and other relevant expenses of the learners.

The tutorial service program will be into group and individual.
