Dear colleague,
we would like to inform you about the above-mentioned patient, who was
undergone the medical treatment in our emergency center:
Principal diagnoses:
1. Currently 01/21: Urinary tract infection
2. Status post Gravida (the number of times the women has ever been
pregnant) IV. Para (number of babies delivered) III, 3 Caesarean sections,
the last one took place a year ago.
Anamnesis:
The presentation of patient took place in our hospital. The patient submitted also a
clinical picture of a urinary tract infection.
The patient has been describing that “Pollakiuria” and “Alguria” have taken place
since the early evening. It is detected that (peripheral) blood contamination does
not exist in urine. Proper stool/bowel movements, last one has taken place this
afternoon. No fever. The patient has not yet had a urinary tract infection.
The last cesarean section took place a year ago, no urological abnormalities.

No allergies are known

clinical problem/issue:
Thyroiditis due to new swelling
Findings:
Sonography of the anterior neck region incl. color coded duplex sonography
(CCDS):
Normal results with respect to thyroid glands parenchyma with regular perfusion in
the color coded duplex sonography (CCDS). The volume of the thyroid glands is
71 ml, on the right side: 6 ml and on the left side: 65 ml.
There are several nodules in both lobes of the thyroid glands, exemp. in the
following sites:
right central apical spongiform: 0.2 ml.
right central apical, hypoechoic with smooth borders: 0.1 ml
left apical conglomerate, inhomogeneous echo pattern: 15 ml.
left central caudal conglomerate, inhomogeneous echo pattern: 37 ml.
no path. enlarged neck – lymph nodes.
normal results with respect to the submandibular salivary glands and mouth base.
