Republic of the Philippines ISO 9001:2015 QMS CERTIFIED INFORMED CONSENT STED LIMITED-FACE-TO-FACE CLASSES This document contains critical information regarding your DESIRE/WILLINGNESS to participate in the Biliran Province State University's (BiPSU) STED Program's Limited-Face-to-Face classes. Our decision is based on the recommendations of the Local InterAgency Task Force (IATF) and the Commission on Higher Education (CHED), in accordance with the provisions of Joint Memorandum Circular (JMC) No. 001, s. 2021. Rationale Mitigation By participating in the university's limited face-to-face classes, you agree to take certain precautions to protect yourself and others from exposure, illness, and possible harm. If you disregard health and safety protocols, you will be denied access to the face-to-face classes. Each protocol below should be read and understood: 1. 2. 3. The face-to-face modality of instruction was the foundation of education students training for many years. Despite advancements in flexible learning education, classroom and hands-on learning remain important components of the educational experience. When simulations are conducted in conjunction with actual demonstrations of pedagogical knowledge and skills in laboratories and applied or practiced in the real world such as teaching in actual classrooms in elementary and secondary, then these competencies are more effectively experienced and absorbed. 4. 5. 6. 7. 8. 9. Challenge Demonstration Teaching is done through the application of the theories of teaching and learning in a classroom setting. Although virtual classes can be done synchronously and asynchronously however students learning and mastery are always hampered by uncontrolled factors such as slow internet connectivity. The conduct of assessments is also better implemented during the face-to-face classes considering that education programs are subjected to the Licensure Examination for Teachers (LET). Although participation in face-to-face classes is not mandatory, the lack of participation in such a modality may result in an education student's competencies being less developed than those who participated in and learned through the face-to-face mode of instruction. You are fully aware that by attending university and leaving your house, you are exposing yourself to the risk of coronavirus infection. This risk is increased if you use shared public transportation such as tricycle, padyak (sikad2x), V-hire, motorcycle, multicabs and jeepneys. 10. 11. 12. 13. Before I could attend the limited face-to-face classes, I must be fully vaccinated. If not from Biliran Province, I must present my Biliran QR Code and Vaccination Card upon entry into the province. When I arrive at the university, I must show my School ID, vaccination card, and QR Code. I must always wear a face mask. I am required to bring hygiene kits and present them at the university's main entrance gate. I must complete a health declaration form that has been issued or secured in advance to avoid crowding the main door while filling out the papers. (All participants are urged to bring their own pens and utilize them). My temperatures will be taken upon entering the University’s main entrance. I am a dependent or a member of PhilHealth or another medical insurance that covers COVID-19-related medical expenses. I am aware that an antigen test is required every two (2) weeks if I am not fully vaccinated but wish to attend the limited -face-toface classes. If I have symptoms when I arrive, I will be transferred to the holding area for evaluation, sent home, or referred to the MDRRMO. If I exhibit symptoms during classes or on campus, I will be transferred to the isolation room for assessment and possible referral to the MDRRMO, or I will be sent home. When I'm not feeling well, I'm supposed to stay at home. When I return to class and after validation from the University Clinic, I will need to present a medical certificate from the municipal RHU.
