First week of the

Students will be sent the needed guidelines
Students will be sent the 100 medications list
Students will be sent the medication history PPP plus the SOAP notes
document for first assignment
BB live meeting Monday January 10th at 1-4
pm
• Discuss the deadlines of the training through the PPP.
• Discuss the materials (Log book, and documents to be filled by
students in Al Baraha hospital and Al Kuwait hospital) sent by Dr.
Mena.
• Discuss any other issues related to timings, etc.
• Go over the Induction session PPP.
• Answer any questions related to deadlines.
• Discuss Taskstream and it’s modalities.

BB live meeting to discuss the medication history taking and SOAP
notes
Students should submit a scenario from a family/neighbor/friend
about his/her medication history and comment on it according to
guidelines. Select a patient with minimum of 3 chronic conditions
including 4 medications minimum (4 pages)
Due date for submission of medication history and SOAP first
assignment: Sunday, January 23rd, 2022
Log book week 1 plus 2 submission is Sunday, January 23rd, 2022

DUR document review and explanation
Students will be sent the DUR document for reference
DUR assignment: Submit a case of a patient from the pharmacy on
polypharmacy and create a DUR (MTM)of his therapy. The DUR can
be prospective, concurrent, or retrospective. (2 pages minimum -4
pages maximum)
Due date of 2nd assignment submission is: January 30th on Sunday ,
2022
LOG Book week 3 submission is Saturday January 30th, 2022.
