1.3.150v6 INSTITUTION OF RAILWAY SIGNAL ENGINEERS LICENSING SCHEME Signalling Test Assistant - Assessment Checklist Applicant’s Name Initial application Phone No. Email Surveillance (at 5 years) Renewal (at 10 years) Scope Statement A Signalling Test Assistant will be responsible for assisting under direction the testing of signalling components and equipment in accordance with the test specifications and plans Units of Competence 1 Undertake and record tests/checks of signalling equipment Completing this checklist: Please see page 2 for the requirements for completing this checklist; note that a self-assessment needs to be completed by the applicant for each of the performance requirements Assessment requirements The assessment requirements are defined for each of the performance requirements; where possible an observation should be supported by the examination of suitable documentary evidence, previously completed work, witness testimony, personal statements or questioning. The assessor is to ensure that the applicant has the correct authorisation for the testing work being undertaken Summary of Workplace Assessment and Assessment Methods Used Professional discussion / questions Review of Applicants statement Examination of products / documentation Observation Witness testimony I confirm that the applicant has met the performance/knowledge requirements and that the assessment has been carried out impartially in accordance with the Licensing Scheme requirements Name Signature Date Summary of Competence Assessment/Surveillance Competence assessor’s comments on applicants logbook I confirm that this assessment has been carried out impartially in accordance with the Licensing Scheme requirements and that the applicant is competent in this licence category. Name Signature Date If this assessment was subject to internal verification please complete below Name Signature Date 1.3.150v6 Page 1 1.3.150v6 INSTITUTION OF RAILWAY SIGNAL ENGINEERS LICENSING SCHEME Signalling Test Assistant - Assessment Checklist Requirements for completing this assessment checklist This checklist may be completed entirely by hand or by typed entry in the “Personal statement”, “Workplace assessment” and “Competence assessment” boxes (including summaries). When typing, the boxes will expand to accommodate the length of the comments. When completing electronically, wherever possible embed the supporting evidence in the marked area; this can be done by clicking on “Insert” from the toolbar menu, then “Object” and on the dialog box select “create from file”, then “display as icon” and “browse” to insert the required file; alternatively hyperlinks can be inserted but they must link to an external data storage that can be passed to the assessor. If completing by hand, the boxes can be enlarged by inserting spaces prior to printing. Self-assessment by the applicant A self-assessment must be completed by the applicant in the box headed “Personal statement (how I meet performance requirements n.n)”. Assessment requirements The assessment requirements are defined for each of the performance requirements. Where it states “Evidence must include observed natural performance” this means that the assessor MUST observe the applicant’s performance for the criteria stated as part of the initial application and at renewal; the assessor should state where the observation took place and on what type of equipment the performance was observed. In cases where simulation is permitted, the applicant must still be observed undertaking these tasks even if this has to be on a test rig or in the training school when particular attention should be taken to replicate as far as possible the environment of the operational railway and the assessor should state how this was achieved. Just asking the question “What would you do if…” may indicate underpinning knowledge but it must not be used on its own since it is not a substitute for observing the applicant’s performance when carrying out the task. Knowledge requirements The candidate is advised to check that they meet the underpinning knowledge requirements listed after the last unit. The workplace assessor (or competence assessor in the case of a single level assessment) must indicate how the candidate has demonstrated they have met each of the underpinning knowledge requirements. Meeting all the performance requirements Where a number of performance requirements are grouped together in boxes, all of these must be met and in the context of the unit title. Surveillance (after 5 years): Workplace assessment is not required In addition to their personal statements the applicant should provide supporting evidence that they have been carrying out licensable work satisfactory for the previous 5 years, cross referenced to relevant entries in their logbook or provide other suitable evidence. Where the previous 5 years has shown satisfactory performance, then observation is not required and the checklist can be completed by an approved competence assessor. Renewal (after 10 years) A single level of assessment is permitted by an approved competence assessor provided they are also approved to carry out the workplace assessment for this licence. In addition to their personal statements the applicant should provide supporting evidence that they have been carrying out licensable work satisfactory for the previous 5 years cross referenced to relevant entries in their logbook or provide other suitable evidence. The single stage renewal assessment must be carried out within six months of the category expiring otherwise both workplace and competence assessments must be undertaken. The “Work experience sheets” from the logbook must show that the applicant has been undertaking satisfactory licensable work over the previous 5 years. The competence assessor must observe the applicant’s performance where this is mandated in the assessment requirements where it states “Evidence must include observed natural performance” when the competence assessor should state how the performance requirements were met, including what was observed. For these performance requirements the competence assessor should use the workplace assessors box ensuring that all the workplace assessment criteria stated has been met Internal Verification The IV to indicate if the assessment has been subject to internal verification. The IV report should be available for inspection at the assessing agent’s annual audit Feedback from applicant The IRSE values feedback as part of its commitment to continuous improvement and would appreciate a few moments of your time to complete a short questionnaire which can be downloaded from the IRSE website; www.irselicensing.org - follow the link to “Assessment Feedback Form” under “Documents Quick Links”. When completed please hand back to your competence assessor or Assessing Agent Manager. Additional instructions for assessors Please ensure that your comments relate to how the applicant met the performance requirements in the context of the unit title cross referencing to any documented evidence. Any questions asked and the answers given may be included in the boxes or clearly cross referenced to a separate Q & A sheet submitted with the completed checklist. . 1.3.150v6 Page 2 1.3.150v6 INSTITUTION OF RAILWAY SIGNAL ENGINEERS LICENSING SCHEME Signalling Test Assistant - Assessment Checklist Unit 1 - Undertake and record tests/checks of signalling equipment 1.1 Access the work locations safely, within any agreed timescales and in accordance with site access procedures You accurately establish the location of the task Examples of locations include: ● External (e g trackside) ● Internal (e.g. signal box, equipment room, off-site etc) ● Areas to which the Public have access ● Confined spaces ●Elevated structures ● Areas with hazardous conditions Assessment Requirements Evidence should be as a result of direct observation if possible, however suitable historic evidence, supported by personal accounts and questioning is acceptable.
