Equipment Deficiency, Major Repair or Improvement Wildland Fire Equipment Operators and Repair Personnel are requested to report a deficiency, major repair, or improvement. A major repair is one that cost $1000 or more. The information reported will be used to help track deficiencies and make improvements to US Forest Service Wildland Fire Equipment. This reporting form does not replace the direction to use the deviation process. Just because a deficiency or improvement has been identified, end-users cannot just make the changes without following the deviation process.
