This is not an actual Costco membership number, but is a Costco Membership Activation Certi cate worth $60 for each card number listed above. A Costco
membership requires completion of a membership application. New members are required to present valid government-issued photo identi cation before signing up
for a membership. Coupons are not generated by purchasing a Costco Membership Activation Certi cate. Coupons will be mailed to you within 2 to 3 weeks of
processing your membership application. Costco Membership Activation Certi cates are not valid for merchandise, membership renewal, or cash, nor is it a debit or
credit card except as required by law.
Visit Costco.com to nd the warehouse location near you. Membership Activation Certi cates will not be replaced if lost, misplaced, destroyed or stolen. Membership
Activation Certi cates do not expire and there are no dormancy fees. Membership Activation Certi cates cannot be used to purchase additional Membership
Activation Certi cates or Costco Cash Cards.
Costco accepts all Visa® cards, as well as Costco credit cards, cash, checks, debit/ATM cards, EBT and Costco Cash Cards.
