1. While studying this module on the Health System and the elderly population, the
lesson I’ ve learned is that we must prepare ourselves that with the improvement of
the health and treatment system, it will also increase the life expectancy of a person
so this will bring a consequence where the number of elderly population is increasing,
which has the impact that more patients with comorbid and degenerative diseases
must be treated. From the module it was conveyed that we must be ready to face this
surge. There is something that needs to be done to make a change in dealing with the
spike in elderly patients. We have to make some changes for example the goal of
health care is to enable a longer healthy life, from hospital care to a just in time logic,
which avoids hospitalization as much as possible and empowers home care by
activating the family as care giver for geriatric patients. Besides that, it also provides
facilities and infrastructure to make it easier to obtain health information for home
care patients and also facilitate access to health services, especially in remote or
peripheral areas. From my opinion, there is no one that is the most important, all of
them are very important and mutually support each other so that the goal of being
able to serve these geriatric patients can be carried out properly. As a physiatrist, I
totally agree with the speakers in the webinar or the writers in the journals convey,
especially in relation to preparing care givers in home care so as to educate patients
and their families to be more independent. In this concept, what influences my view
is a very critical perspective and invites me to change and be more courageous in
making a change in the care of geriatric patients by using technology that makes all
services like in our hand.
2. Important lesson for me, as a physiatrist how can I help geriatric patients to be much
more independent at home care setting and also prepare nurses / care givers to assist
in home and prepare for the services provided by families / nurses at home to maintain
good and safety quality so complications that should not occur can be avoided so these
will brings elderly continue to get healthy and active aging and reduce the number of
hospitalizations. What made me change my mindset is that even with the limitations
that exist in the hospital I work in and in the country of Indonesia where I live, it doesn't
make me stop, but I will start from myself by making small changes by setting
education more enjoyable with patients and families so that they will feel
comfortable and safe even though they have to do treatment at home and also
prepare how the house where they live becomes more friendly to the
elderly. Meanwhile for hospital care related to services for geriatric patients who have
started to make the hospital a friendly hospital for the geriatric by providing ease of
services and also supporting facilities and infrastructure for the geriatric, both
outpatient and inpatient care. What encourages me to do more learning in this field is
because I am a physiatrist and also I belong to the geriatric team at the hospital so I
want to make good changes in the team. For example, what I will do is improve home
care. Together with the team, make education more fun by helping families prepare
what is needed for this elderly care. The convenience provided by the registration
process and also an easy teleconference where all teams are present so that they can
provide complete input.
3. List of Actions I have taken for improvement

Actions
Conduct education, either directly or indirectly, to the patients and their
families more enjoyable and efecttive
Conduct assessments and family conferences regularly
Conducting training for the community village outreach services / programs
Visiting a retirement home or old people’s home
5
Collaborating with primary health facilities
What makes me act different is that I am increasingly convinced that to make it easier in the
future so that geriatric patients can be much more independent in their care at home, we
must prepare and provide good education and also carry out ongoing evaluation / monitoring
by making education more enjoyable and facilitating consultation even though can't directly
can via teleconference .
This is very relevant to my context as a physiatrist where I have to maximize and empower
what is in patients and their families so that healthy aging will be achieved. For example,
having to be treated in a hospital also provides ease of service, especially for geriatric patients
with its complexity, which always brings a geritaric team to always carry out assessments and
family conferences to discuss existing problems.
In my opinion, this module has been very good and has opened my horizons towards geriatric
patient care and also to prepare for the predicted surge in cases in 2030. Especially in the
Indonesia, which from the predictions in the graph shows an increase in the growth rate for
elderly patients, this will make me personally to help make the government's program
success, one of which is geriatrics as a national flagship program. I will participate more and
more in programs related geriatric service and invite the geriatric team to participate more
actively outside the hospital by opening up insight for the community in achieving healthy and
active aging so that they are more confidence and ready to take care elderly which condition
requiring home care setting.
