I want to assure you any information you give me will remain confidential and will only be shared with the Department of Public Health for the sole purpose of preventing the spread of COVID-19. (10 days from onset of symptoms or 10 days after positive test taken) Let me explain what this means. During your isolation period you should stay home and separate yourself from others. Stay home except to get medical care ○ If you do need medical care, be sure to contact your medical provider to inform them of your COVID status so they can provide you with any instructions before your appointment. Typically isolation ends 10 days after the onset of symptoms or 10 days after your positive test was taken, but before you can end isolation you must meet certain criteria. 1. 24 hours have passed since you last had a fever over 100 degrees without the use of fever reducing medication. 2. A close contact is someone who had contact with you during your infectious period. This will include anyone who: ● Lives with you ● Was face to face and within 6ft of you for 15mins or more. The 15mins can accumulate throughout the day such as 5mins and 10 mins in the afternoon. ● Anyone you’ve had direct physical contact with such as kissing. ● Anyone who has came in contact with your secretions such as used tissues Your infectious period starts 2 days before your first signs and symptoms. QUARANTINE INSTRUCTIONS Non-household members ● We will inform your close contacts about their exposure to Covid-19 ● We will not disclose any of your information when informing the contacts Household members ● Will you completely be able to isolate completely from the household member until the end of your isolation period, this will determine the quarantine time. ● Quarantine means that they should try not to have contact with other people, stay home except if they need to see a doctor. ● Wear a mask when they are around others. ● Do not share personal household items such as: cups, towels and utensils. ● They should wait 5 days from last exposure before getting tested, unless they become symptomatic, then they should get tested for Covid-19 immediately. (Incubation period can take from 2-14 days with an average of 5 days) ● Call us if they get positive test or symptoms arise ● Quarantine will begin on the last date to exposure.
