I respectfully inform you that I have got my child delivered at Health And Maternity Hospital on April
19th, 2021. I am writing this application to register my child and also ask for issuance of official birth
certificate. I have attached all the hospital documentation required in this context with the application.
I would like to request you to please process the request for the document at the earliest as I would be
requiring the birth certificate to complete the family insurance documents.
