A. Infant - AR/CPR
1. Survey the Scene. ​“The scene is safe”
2. Introduce self to parents and ask permission.
“Ma’am, Sir, I am ________, a trained first aider. Can I help?”
3. Position yourself properly to the side of victim.
4. Check responsiveness. Tickle foot and palm of hands 3 times.
“Baby unresponsive. Help! Help! Help!”
5. Tilt head slowly and support using two fingers at bony side of jaw.
6. Check for any obstruction in the mouth.
7. Check pulse and breathing. Pulse is at inner arm between shoulder and elbow. Put cheek
close to nose and mouth for 10 sec and check for rise and fall of chest.
“1, 1002, 1003, 1004, 1005, 1006, 1007, 1008, 1009, 1010”
8. “Victim is breathless and has no pulse. Activate medical assistance and report back to me as
I perform CPR”
9. Index finger at nipple, slide opposite to you to the middle chest. Put two fingers (middle and
ring) down and pull the index finger back up. Make sure elbows are level with palm.
10. PERFORM 5 CYCLES OF CPR. 30 compressions, 2 blows after each compression.

13. Check breathing and pulse (Repeat #7)
“Victim is revived”
14. Put the infant in recovery position. Support the back of the neck of the infant and lower butt
as you lift the infant up.
15. Advise the parents to bring the infant to the nearest hospital to check for complications.
A. Adult - AR/CPR

18. Position yourself properly to the side of victim.
19. Check responsiveness. Tap the shoulders of the victim
“The victim is unresponsive. Help! Help! Help!”
20. Tilt head slowly. Base of palm on head, lock elbow on floor, support using two fingers at bony
side of jaw.
21. Check for any obstruction in the mouth.
22. Check pulse and breathing. Put two fingers on the neck of victim. Put cheek close to nose
and mouth for 10 sec and check for rise and fall of chest.
“1, 1002, 1003, 1004, 1005, 1006, 1007, 1008, 1009, 1010”
23. “Victim is breathless and has no pulse. Activate medical assistance and report back to me as
I perform CPR”
24. Place heel of hand at center of chest at lower half portion of sternum. Careful not to hit the
xiphoid. Place hand over the other, interlace. Tilt your palm. Make sure hand is straight and
shoulder perpendicular to chin of patient.
25. PERFORM 5 CYCLES OF CPR. 30 compressions, 2 blows after each compression.

29. Position victim to recovery position. Raise arm next to me, cross legs, put other arm on chest.
Support back of shoulder and side of the hips. Let patient face me.
Child - AR/CPR

38. Tilt head of patient up with one hand. Locate nipple and place heel of hand at center of chest
at lower half portion of sternum. Make sure hand is straight and arms and legs are positioned
properly. Look at the face of patient while doing compressions
39. PERFORM 5 CYCLES OF CPR. 30 compressions, 2 blows after each compression.

Transfer
1. Wash hand or sanitize using 70% alcohol
2. Gather all needed materials (cleanest to dirtiest):
a. Gait Belt (support from bed to wheelchair and goes to the back pocket of wheelchair)
b. Draping (to drape the patient and goes to the back pocket of wheelchair)
c. Wheelchair (park, lock wheelchair
d. Footstool under the bed.
e. Bedpan and urinal (depending on sex; hold at the cleanest part)
3. Sanitize hands again.
4. “Good afternoon. I am _________ your caregiver for today. We will transfer you from here to
the other room.”
5. Provide privacy.
6. Lock the wheels of the bed.
7. Go to the side of the patient and lower side rails.
8. Take vital signs. Make sure it is normal.
9. Offer bedpan. ​“Ma’am, would you like to use the bedpan?”
LEFT HEMIPLEGIC
10. Position wheelchair at the foot side of the bed. Make sure that the hand rail is close enough
to the side of the bed which is about 2 inches. Lock the wheelchair.
11. Get the Gait Belt and introduce it to the patient.​ “I will be putting this around your waist later
to support you as we move you to the wheelchair.”
12. Position the footstool on the bedside.
​Position to Long Sitting
13. Uncover draping. Cross arm on the chest (good over bad) and cross legs together (bad over
good).
14. Move the patient to the side of the bed using the draw sheet. Put arm around shoulder to
support the patient when sitting up and pivot the legs to help the patient to the long sitting
position.
15. Tell the patient to use his or her right arm for support while you put on the Gait Belt.
16. Instruct the patient to inhale. Put the Gait Belt around the waist of the patient and tie the half
knot. Check with the patient if it is too tight. Make sure to give an allowance. Finish half knot.
17. Instruct the client to put her good arm at the back of your shoulder.
18. Hold the Gait Belt on the Bad side. Lock with your thumb on top.
19. Assist the patient to put on footwear.
20. “Ma’am we’re gonna stand up on 3. 1, 2, 3.”
21. “Ma’am please move your right leg to the right leg to the right side. In 1, 2, 3.”​ Push bad leg
toward good leg.
22. Pivot patient right in front of the foot rest of the wheelchair.
23. “Ma’am, I’m going to pivot you to the right. 1, 2, 3.”
24. “Ma’am, please move your right leg backward. 1, 2, 3. *Push bad leg.* Okay another step
backward. 1, 2, 3. *Push bad leg.*”
25. Ask if the patient can feel the edge seat of the wheelchair. Instruct the patient to hold the
handrail with their right hand.
26. “At the count of three, you have to sit down. 1, 2, 3.”
27. Once seated, untie the knot of the Gait belt.
28. Place the feet of the patient on the foot rest. Bad leg first, followed by good leg.
29. Go to the back of the wheelchair. Place your arm across the chest of the patient to help the
patient lean forward as you push the patient on his back while supporting with your left hand.
30. Remove the Gait belt by reaching on the back of the patient. Place the Gait Belt at the back
pocket to remove draping.
31. Help the patient lean backward and remove arm across the patient's chest.
32. Open folded draping and drape the patient properly. It should cover the lap until below the
knee. It should not touch the foot rest of the patient.
33. Remind the patient where she is going. ​“Ma’am, we will now go.”
34. As you approach the door, turn the wheelchair around, lock the wheels, then open the door.
35. Once the door is open, unlock the wheelchair, turn, then walk out of the room.
36. Once out of the room, stop, lock the wheels, close the door. After, unlock wheels then go.
37. When you approach the other room, turn the wheelchair around, lock the wheels, then open
the door. Once the door is open, unlock the wheelchair, turn, then go inside the room.
38. Place the patient on the head side of the bed. 2 inches between handrail and wheelchair.
39. Fix the bed and position the footstool correctly at the bedside.
40. Go to the front of the wheelchair and remove the draping. Fold draping properly.
41. Go to the back of the wheelchair and inform the patient that you will be putting your arm
across her chest. Push slowly at the back as the patient leans forward.
42. Place the draw sheet in the back pocket. Remove the gait belt. Hold at the center of the gait
belt. Inform the patient that you are going to insert it behind the patient’s back. Let the patient
lean backwards.
43. Go to the front of the patient. Do the half knot as you instruct the patient to inhale. Tighten the
finish your tie.
44. Put good leg on the floor. Fold footrest. Assist when putting bad leg on the floor and put it
near the good leg to support the bad leg. Fold footrest. Hold the gait belt on both sides of the
patient. Make sure that the bad leg is knee to knee with yours.
45. Instruct patient to hold at the arm rest or at the back of your shoulder.
46. “At the count of 3, we’ll stand. 1, 2, 3.
Put the right leg forward at the count of 3, 1, 2, 3. *push bad leg*
Another one. 1, 2, 3. *Push bad leg*.
Now, I will pivot you to face your left side. 1, 2, 3.”
47. Make sure the patient can feel the side of the bed. Assist in moving back until she does.
48. “Ma’am, please sit down at the count of 3. 1, 2, 3.”
49. Have her place right arm at the back for support.
50. Remove footwear of patient while still holding the gait belt.
51. Untie gait belt. Place it on the wheelchair.
52. Cross legs of patient. (Bad over good)
53. Pivot patient toward the bed for lying down supine position.
54. Move the client to the center of the bed. Put the cover up.
55. Ask if the patient feels okay or if there are any medical complaints.
56. Take vital signs.
57. Fully cover the patient with a blanket and put the side rails up.
58. Inform the patient that you are done. Endorse the patient to the physician.
59. Sanitize hands and bring the wheelchair with you as you exit.
