Faculty members and students who wish to use audio visual equipment and facilities are responsible to return and request materials at the IMS office. Request for audio visual equipment and facilities are filed strictly at least one (1) day before usage at the IMS office. Reservation by phone is accepted. However, the reservation form must be filled out for confirmation before the date of usage. IMS reserves the right to cancel unconfirmed reservation. Faculty members and student who fail to cancel his reservation at least two (2) hours prior to the reserved time shall be given a one (1) month suspension on the use of audiovisual facilities, materials and equipment for the first offense and one (1) semester suspension for library and IMS privileges for the second offense. Media resources to be used outside the campus are subject to submit Requisition Letter at least (3) working days before the target date and must be signed by the Vice President for Academics and Service and Vice President for Finance and Administration. Others are strictly for campus use only. • The assistance of the IMS personnel in the demonstration of the proper use operation of the equipment may be requested • The availability of the facilities, equipment and personnel must be ensured when filling a request • Faculty members and students may preview materials upon arrangement with the IMS personnel • The “No Smoking Policy” is strictly enforced • Food and drinks are prohibited inside the audiovisual facilities • The borrower must make sure that the facilities are clean and in order after every use. Borrowers are responsible for all the materials or equipment checked out under their names • All reservations must be approved and signed by the IMS Head IMS LOAN REQUIREMENTS • All borrowers are required to surrender their borrower’s ID upon borrowing of materials and/or equipment. Using somebody else’s ID is subject to disciplinary actions. • All reservations must be approved and signed by the IMS head. RETURNING OF BORROWED MATERIALS • A borrower’s card is required for every transaction. • All media equipment are strictly for campus use only. • All equipment must be returned fifteen (15) minutes before the library closing time. INTERNET SECTION POLICY The Olivarez College information Resource Center (OCIRC) is pleased to offer its clientele access to a wide array of resources on the Internet. We would like to remind the users of this service that: Most of the personnel have no control over quality of information on the Internet. Each user must consider the validity and reliability of the information illegal. Hence, use of the Internet must be compatible with the mission of the OCIRC. It is the responsibility of each user to be guided by the following principles: INTERNET SECTION POLICY • Internet Section is open 8:00 am to 8:00 pm Monday to Friday and 8:00 am to 12:00 nn every Saturday; • Only clientele with validated Borrowers Card are allowed to use the computer; • The computers may be made one (1)day before the desired schedule. (Reservations not claimed in 15 minutes of the reserved time may be forfeited in favor of other users) Clientele without reservations may be accommodated unless there are computer terminals available; • Only one (1) user is allowed per computer terminal; INTERNET SECTION POLICY • If a computer malfunctions or have evidence of a computer virus, notify the computer technician or student assistant immediately; • Wi-Fi service for personal computers is available; • Respect for the privacy and sensibilities of other users; INTERNET SECTION POLICY Further examples of UNACCEPTABLE USE include, but are not limited to, the following: • Damaging or destroying equipment, software, or data belonging to the OCIRC or to other users, including adding, altering, or deleting files on OCIRC workstation hard drives or other computer equipment; • Unauthorized copying of copyrighted material; • Violating computer system or network integrity, including attempts to by pass network security functions, obtain passwords, or alter the configuration of OCIRC workstations in any way; INTERNET SECTION POLICY • • • • • Harassing others; Using the internet for any illegal activity; Gaming on the Internet; Saving data files on computer hard disks; Eating and sleeping inside the Internet Sections; • Wasting finite resources (including printing without paying). INTERNET SECTION POLICY Violation of this policy may result in loss of Internet access indefinitely. Clientele who make unlawful use of the Internet are subject to investigation. RULES AND REGULATIONS GENERAL RULES • Every student is required to have a borrower’s identification card. • OCIRC users should submit their belongings in the depository upon entering the library except for valuables or fragile things. The OCIRC shall not responsible for any loss or damage. • Library users should submit their belongings for inspection before leaving. • Students entering the library should be properly dressed. • Silence must be observed strictly. • Children below 10 years old are not allowed to enter the library. • Loitering, littering and, sleeping are strictly prohibited. • The OCIRC must be kept clean and orderly at all times. • All communications devices must be switched off or set on to silent mode while inside the library. • Handle books with extra care. • Return newspapers and magazines in their proper places. • The librarians and staff are directed to enforce silence as well as implement the rules and regulations. Fine and suspension of library privileges for one (1) semester. Fine and suspension depending on the gravity of the delinquency. LOSSES Any loss of library materials should be reported immediately to the Reserve Section. Borrowers should pay the replacement cost and processing fee of one hundred pesos (P100.00) and overdue fine if the item is lost or damaged. THEFT Theft of library materials is a serious offense resulting to a penalty of one thousand pesos (1,000 in addition to replacement costs. Other penalties include academic sanctions and suspension of library privileges and disclosure of the offense to academic chairperson concerned. Security officers may also be called to assist in cases of theft. VANDALISM, DAMAGES AND MUTILATION Borrowers may also be charged for mutilation, vandalism or damage to library materials while on loan to them. Charges for damaged items vary depending on the extent of damages (ranging from the cost of rebinding an item to full replacement cost). An administration fee is charged in addition to any replacement or repair cost. Mutilation will be penalized by paying the full cost of the book and the processing fee of P100.00. FORGERY Forging the librarian’s signature for any purpose shall be penalized by a suspension from library privileges. Other penalties include academic sanctions and Disclosure of the offense to academic chairpersons concerned.
