Team Peer Assessment Form
Fill in the names for each member of your team (including yourself), and beside each one give a score for
that person’s participation in your group. Give 100pts if a person was willing and able to do a fair share
of the group work. Most people should receive 100pts and many groups give all members 100pts.
However, if someone did not fully participate with the group you may award them something less than
100pts. Once you complete the form, upload it into the assignment in Canvas. You should probably need
additional room, please feel free to add rows; you may also delete the example, and these instructions.
In addition to a scoring number, for full marks you must include specific, informative and insightful
remarks for yourself and on each teammate. Conduct this as you would an appraisal in real life. Feel
free to use more space. You may delete the instructions and example as well, if you wish.

Was great at keeping us on track in meetings, but Very Good Analysis
missed a couple of key deadlines. Didn’t respond Full marks = A+
to teammates’ texts or emails in a timely fashion.
In fact, she disappeared for three weeks in the
middle of the semester. Is an excellent writer and
formatter in PowerPoint. Had she been more
accessible to the group, I would have given full
marks. Was really good at identifying problems
and proposing solutions before we went down
the wrong road too far. If I were her manager, I
would want her to work on x, y, and z to
complement her strengths of a,b and c.

I mostly helped out and tried to pick up the slack
when needed. I feel my strengths were
organization and meeting deadlines, while I could
have been more punctual to team meetings, and I
missed a group mentor meeting. My team was
really strong and we worked well together; my
role was one of keeping the team on track.
He really put in a lot of effort, especially at the
end of the project. Overcame a slow start to really
produce, etc. Wonderful teammate, put in extra
work to make sure we stuck to deadlines, always
pushing us to keep up, a true leader, etc
Usually missed deadlines, hard to contact, didn’t
respond to emails, didn’t attend meetings,
Argumentative with teammates, poor quality, etc.
Did everything assigned, but seemed distant and
never contributed without us asking.
