1.2 FAMILY BACKGROUND
Summer is only child. Her mother works for them and
send Summer in school. Her mother need to support her aunt
in sending to school that is why Summer did not attend her
therapy session anymore.
II.
MEDICAL BACKGROUND
During Miss Locsing’s pregnancy, she went to normal
delivery with Summer. Other than that, all seems to be ok while
growing up.
III.
ASSESSMENT
COGNITIVE SKILLS
In Language, he’s familiar with the alphabets and her first
language is English.
In Numeracy, she is knowledgeable of shapes, colors, numbers 150 and rote counting from 1-50 as well.
In Science, she knows and can name the body parts like eyes,
nose, ears, mouth, teeth, hands, head, feet, knees, legs,
shoulders etc.
PSYCHOSOCIAL SKILLS
 She expresses her feelings about herself like happy, sad,
anger, etc.
 She plays with kids in a group but has difficulty in sharing
toys with other children
 She imitates adult activities if she’s interested
 She tells what she wants to do and does it.
 She always like to be praise all the time.
 She is very sweet kid.
INDEPENDENT LIVING SKILLS
Summer performs the independent living skills. She can eat
on her own, dress and undress with buttons, brushes her teeth,
and partially toilet-trained and move her bowels with minimum
assistance.
RECOMMENDATION
 Summer should continue her Occupational therapy
sessions to improve her social interactions, cognitive,
adaptive and executive function
 Increase her Speech Therapy sessions to improve her
higher order language skills, comprehension, utterance of
words and social conversation.
 Do language stimulation activities daily. Encourage
reading of books.
