The paramedic reporting on duty in the am will sign the drug audit sheet for the box he/she will be using that shift. This indicates a box that is properly sealed on your arrival and/or end of your shift. IV fluids for the IV box should be obtained via filling out a requisition form and taking a copy of the PCR with the proper signatures to the pharmacy. Be sure the requisition is complete and accurate and that you have all of the proper signatures on the PCR form. Paramedics, when giving a narcotic and all of the medication is not given, you must waste the remainder in the emergency department with a witness (RN or physician) and obtain their signature on the PCR with documentation indicating what medication and how much was given and how much was wasted. If you decide to give the remainder of the medication to the nurse taking care of the patient for possible use later, you need to obtain the nurses signature as receiving the medication with documentation showing what medication and how much was given by and how much you left with the nurse to use later with he patient. I don’t think we can do this? If you open your box or while in the process of giving a medication you break a vial/ampule of medication, you will need to take the broken vial/ampule along with your requisition to the pharmacy to obtain a replacement.
