AFFIDAVIT OF LOSS
The undersigned, MR. DOMINGO SANGABOL, of legal age, Filipino Citizen, Single and
with office address at Unit 1107 Tower B, Palm Tower Condominium, 7706 St. Paul Road,
Makati City, after having been duly sworn to in accordance with law, hereby deposeth and
sayeth;
That I am the Accounting & Finance Officer of Exodus Broadcasting Company Inc.
with office address at Unit 1107 Tower B, Palm Tower Condo, 7706 St. Paul Road,
Makati City and our company maintain a Current Account with the Landbank of the
Philippines –
That as such, we were issued a passbook covering
our account thereat;
That the said passbook issued to us was lost
misplaced due to circumstances beyond our control, not
of our liking fault or negligence;
That I exerted all efforts to locate the same but
to no avail and the said document is now beyond
recovery;
That I make this statement for the purpose of
inducing the cooperative to issue a new passbook which
will automatically cancel and supersede the lost one;
and if the lost passbook is found, I hereby undertake
to return it immediately for cancellation;
That I am executing this affidavit to attest the
truth and correctness of the foregoing statements for
all legal intents and purposes.
FURTHER, affiant sayeth naught.
