COMES NOW the Petitioner and before this Honorable Court respectfully states that:
1.
Petitioner is of legal age, Filipino, married to Arcely Pajarillaga, and a resident of Turod Norte, Cordon, Isabela where summons and other legal processes may be served;
2.
Petitioner is the registered owner of a parcel of land situated in the Barrio of Camudo, Municipality of Cordon, Province of
Isabela, covered by and described in Transfer Certificate of Title
No. T-127961 of the Registry of Deeds of Isabela. A photocopy of
TCT T-127961 is hereby attached as “Annex B” and made an integral part of this petition;
3.
The original of the aforesaid Transfer Certificate of Title on file with the Registry of Deed of the Province of Isabela was lost or destroyed when petitioner and his family have moved and changed residence sometime in 1990. The Affidavit of loss is hereby attached as “Annex B” and made an integral part of this petition;
4.
Petitioner is in actual possession of the subject parcel of land;
5.
There was no deed of other instrument affecting the property has been presented for registration or if there be any, the nature thereof, the date of its presentation, as well as the names of the parties and whether the registration of such deeds or instruments,
6.
The subject property is free from any and all liens and encumbrances. is still pending accomplishment;

WHEREFORE, the petitioner respectfully prays unto this Honorable Court, after due publication and hearing, to ORDER and DIRECT the Register of Deeds of the Province of Isabela to reconstitute Transfer Certificate of Title No. T127961 in the registered name of the owner in a manner exactly in the same terms and conditions on the basis of the copy of the said title issued to petitioner and a copy of the Register of Deeds. Petitioner also prays for other reliefs which may be deemed just and equitable.
