Ethical, it’s just the right thing to do…
We cannot drill wells without people, the drilling rigs do not operate by themselves.
Want to be proactive, not reactive
Past 2 years we sustain more serious injuries in the last 3 months of each year. (show
graph to group)
Ask group what they think the reasons this happens…get them to participate
Suggestions to get conversation going with group…
• Holidays (Ramadan, Eid, National Day, Second Eid)
• Losing focus (missing the family/friends)
• Self induced stress?
– We want to change history, not repeat it!!!
Based on answers, ask “What do we need do differently?”

1. Incident Overview
Rig roustabout was asked by the night tool
pusher to drive to the camp to pick up the
welder, about halfway to camp the car was
involved in a rollover this occurred on a corner
in the road which resulted in damage to the roof
and left side of the car. The weather at the time
was foggy .

• Speed

• An inexperienced driver
• Speed and careless driving.
• Poor visibility due to uncommon fog.
• Inadequate supervision

company vehicles.
Supervisors to hand keys only to authorised
drivers
Traffic laws and speed limits to be adhered.
Road condition should dictate speed at
which the drivers should travel.

1. Incident Overview
lead floor man and the other hand tried to make
8 ½ bit with mud motor. By lifting bit by hands
one of the hands thought the bit already
screwed in and left the bit At that time the bit fell
down and the injured person could not hold by
himself the bit failed down causing pinched his
right ring finger between bit breaker and bit.
2. Direct Causes
• The mud motor was not vertical.
• Floor hand decide to screw the bit by hand to avoid
cross threads and damaged the bit and motor.
4. Actions to Prevent Recurrence
• The weight of the bit it’s over man capacity.
•
A safety meeting was held with all crews to
discuss understanding of JSAs, procedures
and safety work instructions applicable to
the specific tasks
• Job planning needs improvement.
•
Mud motor need to be vertical not bent.
• Inadequate work instruction.
• No communication between crew.
• Lack of supervision.
•
Communication system will be improved.
•
Supervision of the employees.

• Safety is everyone’s responsibility, both team and
individual level!
• Stay focused, keep your mind on the activity around you!
• Proper planning not only saves time, but saves injuries!
• Take time-out for safety when the scope of work
changes!
• Be Proactive, not reactive! (observations, near miss
reporting, interventions)
• Let’s change history together and finish 2008 without
injury!
