Ethical, it’s just the right thing to do… We cannot drill wells without people, the drilling rigs do not operate by themselves. Want to be proactive, not reactive Past 2 years we sustain more serious injuries in the last 3 months of each year. Incident Overview Rig roustabout was asked by the night tool pusher to drive to the camp to pick up the welder, about halfway to camp the car was involved in a rollover this occurred on a corner in the road which resulted in damage to the roof and left side of the car. The weather at the time was foggy . 2. Direct Causes • Speed 3. Root Causes • An inexperienced driver • Speed and careless driving. • Poor visibility due to uncommon fog. • Inadequate supervision Oxy Oman – Drilling Department 4. Actions to Prevent Recurrence • • • • Only authorised drivers are to drive company vehicles. Supervisors to hand keys only to authorised drivers Traffic laws and speed limits to be adhered. Road condition should dictate speed at which the drivers should travel. 4 Timeout for Safety - December 2008 Oman Rig 862 2 Oct 2008 MTO 1. Incident Overview lead floor man and the other hand tried to make 8 ½ bit with mud motor. By lifting bit by hands one of the hands thought the bit already screwed in and left the bit At that time the bit fell down and the injured person could not hold by himself the bit failed down causing pinched his right ring finger between bit breaker and bit. 2. Direct Causes • The mud motor was not vertical. • Floor hand decide to screw the bit by hand to avoid cross threads and damaged the bit and motor. 4. Actions to Prevent Recurrence • The weight of the bit it’s over man capacity. • A safety meeting was held with all crews to discuss understanding of JSAs, procedures and safety work instructions applicable to the specific tasks • Job planning needs improvement. • Mud motor need to be vertical not bent. • Inadequate work instruction. • No communication between crew. • Lack of supervision. • Communication system will be improved. • Supervision of the employees. 3.
