UMMARY AND
ACKGROUND
As part of the ambulance fleet replacement and reserve plan, the Department of
Emergency Services is seeking competitive bids for the purpose of purchasing (1) 2016 4x2
Chassis, Ambulance Module, with the option to purchase additional ambulances within the terms of the bid.
With the general specifications and pictures provided, the intent will be to closely match the same compartment configurations and lettering of our current 2015 EMS 20 Ambulance module.

ROPOSAL
UIDELINES
This Invitation to Bid represents the requirements for an open and competitive process.
Proposals will be accepted until 4:00pm EST November 23, 2015. Any proposals received after this date and time will be returned to the sender. All proposals must be signed by an

official agent or representative of the company submitting the proposal. All proposals will be delivered sealed. Faxed or emailed proposals will not be accepted.
If the organization submitting a proposal must outsource or contract any work to meet the requirements contained herein, this must be clearly stated in the proposal. Additionally, all costs included in proposals must be all-inclusive to include any outsourced or contracted work. Any proposals which call for outsourcing or contracting work must include a name and description of the organizations being contracted.
All costs must be itemized to include an explanation of all fees and costs. Pricing must include all components for testing, inspections, and freight and delivery charges. The bidder’s failure to include all itemized costs to perform this contract will be the sole responsibility of the bidder.
Sales tax, if applicable (Maryland Sales Tax Exempt) shall be excluded on the initial bid.
Proposed pricing shall be firm from the beginning date of the signed contract. Bidders will also provide proposed pricing for the two-year period following the initial contract, with the expectation that this option will be written into the initial contract.

After the date and time established for receipt of proposal, any contact initiated by any proposer with any Queen Anne’s County Department of Emergency Services representative, other than the Purchasing Agent for the Department, concerning this Request for Proposal, is prohibited. Any such unauthorized contact may cause disqualification of the bidder from this procurement transaction.
Vendors will utilize the QAC DES form, Appendix B as the cover page for the proposal submission. This form will also serve as the evaluation template for the bid review process.
Contract terms and conditions will be finalized upon selection of the winning bidder for this
RFP. All contractual terms and conditions will be subject to review by the County Attorney and will include scope, budget, schedule, and other necessary items pertaining to the project.
All bids shall include one (2) printed copies of each of the following documents : a.

Full size engineered construction drawings. Submitted drawings must be specifically for proposed ambulance and depict all major specified components within the project scope. Submission of “similar to” or
“standard” drawings, or statements referencing submission of drawings after award of contract, will disqualify the bid. These drawings shall show the following: i.
These drawings shall show the following minimum views: front view; street side with proposed chassis; curbside with proposed chassis; rear view; top view with proposed chassis; patient compartment front, street side, curbside, rear, ceiling, and floor layout from above.

The drawings shall contain dimensions for the overall length (in inches), overall height (in inches), wheelbase, angle of approach, angle of departure, overall width of the ambulance and interior dimensions of all cabinets.

All bids will be required to meet the following criteria, inclusively: b) Vehicle will meet current Triple K regulatory requirements at time of delivery, and be labeled as such, to include height and weight specifications; c) The bidder will provide a pre-build meeting and final inspection meeting at the manufacturing facility; a.
