CARONDELET HEALTH NETWORK – Recent Updates Population Health Management • Carondelet Health Network is moving toward a model of care that focuses on population health management. • Population health management, as defined by Carondelet, is the ability to keep healthy people well, identify and “course correct” those who may be at risk, better manage those who are chronically ill, and work to eliminate gaps in care. • This effort would enable Carondelet to help lower the ever-rising cost of healthcare and create a overall healthier community at the same time. • To make this happen, Carondelet is collaborating with Healthways, the largest global provider of well-being improvement solutions—serving 40 million people across four continents. Carondelet Heart & Vascular Institute • In 2013, Carondelet is investing $17 million to build the Carondelet Heart & Vascular Institute (CHVI) on the campus of St. Mary’s Hospital. • The vision for the Institute is to become nationally recognized as a premier destination for modern cardiovascular care through clinical excellence, research, and physician education. • The work of CHVI will not only be evident to patients at St. Mary’s. Many of CHVI’s state-of-the-art cardiology capabilities will also be available to patients on the campus of Carondelet St. Joseph’s Hospital as well. Sahuarita Wellness Pavilion • Carondelet Health Network and the Rancho Sahuarita Company proudly announced in December plans to develop a health and wellness pavilion for residents in Sahuarita, Green Valley and other growing communities along Interstate 19. • This health pavilion concept is the result of several years of collaboration between Carondelet leadership, physician partners, local community leaders, residents in Sahuarita and Green Valley, Carondelet’s Board of Directors and Rancho Sahuarita. • The health and wellness pavilion will initially offer residents along I-19 a wide array of primary and specialty care services. The size of the initial complex will allow Carondelet to double the number of primary care physicians serving the Sahuarita area. • Additionally, the pavilion will also serve as a satellite location for cardiovascular specialists from Carondelet Heart & Vascular Institute and neurologists from the Carondelet Neurological Institute. • Carondelet’s plan will allow for convenient access to these specialists and many other medical specialties in Sahuarita. This will ensure patients receive preventive, diagnostic and follow-up care close to home. Chronic Pain Management Program (CPMP) • Concerned about a dramatic increase in the abuse of narcotic prescription drugs in the Tucson area, Carondelet has instituted a new program in its Emergency Departments (ED) designed to address the problem. • It’s an intensive program designed to tackle the reported 37 percent increase in deaths in Arizona in the past few years from abuse of narcotic prescription drugs. • In his annual report for 2012, Dr. Gregory Hess, Chief Medical Examiner for Pima County, points to a 12 percent increase in overdose deaths in comparison to 2011. Many of those overdose deaths are the result of prescription opiate drugs. • When a patient appears at a Carondelet ED complaining of pain, he or she will receive an appropriate medical evaluation to rule out an emergent medical condition. If the patient repeatedly presents with chronic pain issues, they will be enrolled in the program and each time they come in seeking pain medications for non-emergent conditions, their dosage of narcotics will become more limited and they will be provided support to identify a primary care physician to management their chronic pain. • In reaction to the rising number of deaths in the community from narcotic drug abuse, this program and commitment by Carondelet will also help to reduce the number of narcotics on the street and make our community safer. The Chapel at St. Joseph’s Hospital is truly the centerpiece of the recently completed Fred & Olga Pace Spiritual Center, which includes an all-faith meditation room and healing gardens. • More than 300 individuals, including Carondelet associates, local business and civic leaders, and organizations, donated toward the development of the new chapel after the previous site was razed to make way for the construction of the hospital’s new medical pavilion. • Much of the custom, stained glass windows from the earlier chapel were restored and used in the new chapel, while a new, 8-foot window was commissioned from the same artist to adorn the meditation room. The Breast Center • St. Mary’s equipped its Breast Center surgeons with a leading-industry tool – an intra-operative digital specimen radiography system called the Trident. • The Trident allows surgeons to confirm if an entire lesion was excised by immediately viewing x-ray images of breast specimens. • The technology: Improves patient safety by reducing the amount of time the patient is under anesthesia, reduces procedure time and costs to patients, complies with new accreditation guidelines from the College of American Pathology and the American Society of Clinical Oncology, and decreases the turnaround time for surgeons to visualize the tissue.
