HUMAN RESOURCE MANUAL Office Uniform Subsidy Manual Issue Date March 15, 2011 Effectivity Date March 31, 2011 Version No. 1 Document No. HRM 2.14 Revision No. 0 Page No. 1 of 2 1.0 OBJECTIVE To define the guidelines in coverage, conditions and procedure when granting subsidy on office uniform. 2.0 SCOPE This covers office uniform of regular employees under Operations Group of TSPI Development Corporation (TSPI). 3.0 GUIDELINES 3.1 TSPI considers wearing of uniform as an important element to project its organization’s image – that is efficient, orderly and professional. 3.2 Regular employees in the operations group covering the position titles – sector manager (SM), area manager (AM), branch manager (BM), senior account officer (SAO), account officer (AO), agricultural account officer (AAO), project officer (PO), insurance officer (IO), branch accountant (BA), accounting assistant (AA) and accounting clerk (AC) – are given an office uniform and thus, required to comply with the policy. 3.3 Issuance of office uniform shall be done every quarter. 3.4 The subsidy shall be fifty percent (50%) of the cost of the office uniform. Employee shall keep his employment with the organization for at least six (6) months from the date of issuance of the office uniform. Otherwise, he shall pay the subsidized portion. 3.5 Employee’s share shall be deducted from his salary in 2 equal deductions. 3.6 The cost of the uniform, i.e., in cases of damage, additional sets, beyond the subsidy shall be shouldered by the employee. 3.7 The office uniform is a polo jack of different colors and is worn using the following schedule: COVERAGE COLOR SCHEDULE SM, AM, BM White Monday to Thursday AO, AAO, IO, PO, SAO, BA/AA/AC Yellow Monday AO, AAO, IO, PO, SAO, BA/AA/AC Maroon Tuesday AO, AAO, IO, PO, SAO, BA/AA/AC Light Green Wednesday AO, AAO, IO, PO, SAO, BA/AA/AC Blue Thursday 3.8 Friday is considered a wash day but employees are required to wear appropriate office attire. 3.9 Non-compliance with the schedule is subject to disciplinary procedure under the Employee Discipline, to wit – First Offense – Oral Reprimand Second Offense – Written Reprimand Third Offense – Suspension for 1 day Fourth Offense – Suspension for 3 days Fifth Offense – Suspension for 5 to 30 days HUMAN RESOURCE MANUAL Office Uniform Subsidy Manual Issue Date March 15, 2011 Effectivity Date March 31, 2011 Version No. 1 3.10 Document No. HRM 2.14 Revision No. 0 Page No. 2 of 2 Sixth Offense – Termination from employment Note: Disciplinary process shall be followed based on HRM 4.0 Employee Discipline. Pregnant women are exempted from wearing the prescribed office uniform. 4.0 PROCEDURE Responsibility Immediate Superior Activity 1. Secures a copy of the Uniform Request Form and accomplishes it. 2. Forwards the signed Form to the Human Resource Management Services (HRMS). HR Specialist 3. Consolidates uniform requests and forwards original Uniform Request form to Administration Department. Administration Department 4. Canvasses, selects and coordinates with office uniform supplier. 5. Distributes office uniform. 6. Maintains inventory of un-issued office uniform. 7. Provides Accounting Department with a list of employees who received office uniform for payroll deduction. 8. Provides copy to HRMS for monitoring of request. Payroll 9. Deducts from employee salary his share following the schedule of deduction. Immediate Superior 10. Ensures compliance of employees with the schedule of wearing office uniform. 11.
