To everyone’s surprise —
including her doctor’s — she’s having a heart attack.
Just months earlier, an angiogram had failed to detect signs of
coronary artery disease. How could
this be?
Research is finding that women
and men are different, right down
to the cellular level. And when it
comes to cardiovascular disease,
those differences are proving more
fatal to women.
Women’s Heart Health at Stanford is working to change those
numbers. The clinic, which opened
last year, offers advanced cardiovascular clinical care and is conducting research on gender differences
in heart and vessel disease.

Women do not need a doctor’s
referral to come to the clinic, which
assesses a woman’s risk for heart
disease, and does diagnostic evaluations, risk management and treatment of cardiovascular disease.
The clinic also has a referral
network that includes a dietitian,
a cardiothoracic surgeon, a psychiatrist, a vascular surgeon, an
endocrinologist and a gynecologist
who provide gender-specific care.
Women’s Heart Health offers
online risk assessment, a newsletter
with access to the latest research,
and lectures by top cardiologists on
gender-specific heart disease.
Women’s cardiovascular disease
is called “the silent epidemic,” Dr.
Hannah Valantine, senior associate
dean of Stanford School of Medicine and clinic co-director, told a
packed room at Stanford’s Arrillaga
Center on Feb. 7. An estimated 32
million American women per year
— one in five — have cardiovascular disease.
Women have traditionally been
treated based on research that has
been done on men, according to
Dr. Valantine. While death rates
from heart attacks and strokes have
steadily gone down for men, the
numbers haven’t dropped significantly for women, she said.
“Cardiovascular disease is the
leading cause of death, with 500,000
deaths annually. Fifty percent of
those deaths are from coronary
artery disease,” she said.

only begun to come down recently

Dr. Valantine said.
The reasons for the discrepancy are
myriad. Obesity spiked in the United
States in the 1990s, especially among
women, and obesity and diabetes rose in
African-American women and Latinas.
Gender differences occur throughout
cardiovascular disease and its diagnosis
and treatment, Dr. Valantine said.
Women have delayed referrals when
they complain of chest pain, are treated
less aggressively and receive less diagnostic testing than men, she said.
Detection problems
Part of the problem is that diagnostic
testing doesn’t always show the damage
inside the body. Clinical Director Dr.
Jennifer Tremmel, an interventional cardiologist and researcher, has found that
women with normal angiograms can
have microvascular disease that consists
of blocked smaller arteries and veins not
detected through conventional testing.
Women also can have vascular functional abnormalities, such as endothelial
dysfunction, in which cells in arteries constrict when they are supposed to dilate, she
said. It is not known if these same abnormalities affect men, she added.
Dr. Valantine said 50 percent of women
who have coronary-artery disease show

minimal or no blockage in angiograms,
compared with only 17 percent of men.
And younger women fare the worst,
with those under 50 having a mortality
rate double that for men in the same age
group. Women with diabetes, hyperlipidemia (low HDL, high triglycerides
and abdominal body fat), or those who
smoke also fare worse than their male
counterparts, she said.
Diabetes markedly increases the risk of
heart disease in women over men. The
risk factor is five to seven times greater
in women compared with an increase of
two to three times in men, Dr. Valantine said. And researchers have found
that risks for women with pre-diabetes
is almost as bad as for those with fullblown diabetes — but not so in men.
The classic symptom of a heart attack —
major crushing pain in the chest — is not
the same for women, Dr. Valantine said.
Women frequently experience pain in the
jaw or shoulder (58 percent), sweating (38
percent), nausea (29 percent), shortness of
breath (29 percent), indigestion (21 percent)
and weakness and fatigue (8 percent).
Good news
Not all the news is bad. Although women have increased mortality from stent
implantation to open clogged arteries,
new technologies that put medications
on stents to keep plaque from reforming
are helping to lower the risks.
Cardiologists are beginning to explore
the role of depression and inflammatory
response in cardiovascular disease, Dr.
Valantine said.
The best way to prevent a heart attack
is through measures such as exercise,
diet, and lifestyle changes — and to start
early in life, Dr. Valantine said.
The clinic offers an online Cardiovascular Sex Differences Monthly Journal
Club, an electronic newsletter of research
on gender differences in cardiovascular
disease. And women can take an online
risk assessment through the program or
click on the link “Ten questions a woman
should ask her healthcare provider” to
access the American Heart Association’s
heart-attack risk assessment.
Women must be proactive in addressing heart disease, Dr. Valantine said.
To run-of-the-mill treadmill tests, Dr.
Valantine said to just “say no.” Women
should push their doctors to do treadmill tests that are enhanced by imaging
techniques, she added.
But if symptoms of a heart attack come,
women should not waste time; they
should seek emergency help, Dr. Valantine stressed. Studies show that as many
as 90 percent of women delayed going to
the hospital when they had symptoms of
heart attack, increasing their risk of death.
Self-doubt, embarrassment and denial
were among reasons for the delay.

he small green invitation
printed on recycled paper is
a clue: Menlo School’s 58th
annual benefit, to be held Friday,
April 18, at the Atherton school,
will reflect many shades of green.
Over the years, Menlo School
benefits have “Hit the Road,” been
“Unforgettable,” or headed off to
“Camp Menlo.” This year, “Menlo on the Green” means handling
benefit reservations and holding
an auction online, and dining in
a garden of eco-friendly decor.
By going green, Menlo is also
celebrating the good times on the
green quad of the Menlo campus, as well as honoring seniors
who started as green freshman,
but now are ready to move on to
greener pastures.
The green theme also centers
on a celebration of spring, nature,
and earth-friendly lifestyles.
The benefit features a luncheon
and “senior-powered” fashion
show with an encore performance
at dinner, a live auction, and
dancing to DJ Lou Louben.
For the 12th year, Bloomingdale’s at Stanford Shopping

“Menlo on the Green” will
showcase the talents of Alex Perez,
Jan Chandler, Jeffrey Adair and
BB Bernheim.
Alex Perez, producer, is the
school’s creative arts director. He
has worked as director, choreographer and performer in numerous Bay Area productions, with a
stint on Broadway. Jan Chandler,
who directs the dance segments of
the show, has served as its choreographer for more than 25 years.
Jeffrey Adair, co-owner of J Floral
Art, is returning to coordinate the
fashion show. He has produced or
directed the Menlo show at various
times for 19 years. BB Bernheim,
owner of “BB Productions, is once
again teaming up with Mr. Adair to
produce the fashion show.
The online auction offers opportunities to bid on mentoring by
Bay Area companies, “authors and
speakers” luncheons, fine wines,
and a “count me in party” for 250.
The luncheon and dinner will
be catered by McCall Associations of San Francisco. The luncheon begins at 11:30 a.m. Dinner,
a green-tie/black-tie event, will
begin with cocktails at 6 p.m. with
dinner served at 7 p.m. Highlights
will be a donation drawing for the
grand prize of a 2008 Honda Civic
Hybrid from Anderson Honda
and a live auction.
“M on the Green” is open to the
general public. Tickets are $85 for
the luncheon and $175 for dinner.

M-A senior triumphs over adversity

hatever is ahead for
Rosa Alvarez, a senior
at Menlo-Atherton
High School and a resident of
Menlo Park, it would have to be
rather harsh to outweigh what
she’s already endured. The Family Service Agency of San Mateo
County is recognizing Rosa and
nine other county residents
“who have overcome extreme
personal obstacles to succeed
academically and to become role
models for their peers.”
The 13th annual Winners
Breakfast will honor Rosa, six
other high school students,
and three adults at the Hyatt
Regency hotel at the San
Francisco airport at 1333 Bayshore Highway in Burlingame
from 7 to 9 a.m. on Thursday,
April 10.
Rosa’s older sister Maricela is
gone from her life, the victim of
an unsolved murder case in San
Jose in November 2003. That
tragedy added hardship to a life
already complicated by family problems such as alcoholism
and domestic violence in their
one-room Menlo Park apartment, according to a statement
from the Family Service Agency
of San Mateo County.
Rosa’s father works in construction but is currently unemployed, Rosa told The Almanac.
The family came here from
Mexico when she was in the
sixth grade and could not read
or speak English. Her mother,
who takes in babysitting, still
suffers from depression and
anxiety that came on after her
daughter’s murder, Rosa said.
With babies in the room, was
it hard to study? “It’s kind of
crowded every day and so it’s
kind of difficult,” Rosa said.
She apparently managed to
concentrate anyway, and learn a
second language. She’s taken an
advanced-placement course in
English literature as well as in
Spanish language and Spanish
literature, she said.
Rosa spent a month in Mississippi last summer in a community still recovering from Hurricane Katrina, where she helped
build a pedestrian crossing over
a creek, painted a house, and
helped paint a mural, she said.

Former Woodsider formulates
calcium-rich fruit tea drinks

T
he USDA recommends a daily intake of more than 1,000
milligrams of calcium a day;
that’s three and half 8 oz. glasses of
milk, a pound of almonds, or almost
two pounds of cooked greens.
Dr. Jon Dickinson, a partner in
the California Pacific Orthopaedics & Sports Medicine practice
in San Francisco and a former
Woodside resident, has developed
what he thinks will be an alternate
source of calcium: fruit tea.
Dr. Dickinson and his business
partner, Wyck Hay, a co-founder
of Celestial Seaonings, have created Osteo, a juice and tea beverage that nutrition labels claim
contains 50 percent of the USDA
recommendation of calcium.
In his orthopaedic practice,
Dr. Dickinson has seen many
patients with injuries or conditions stemming from weakened
bones, which he attributes to
calcium deficiencies.
“Osteoporosis is a big problem in the U.S., an emerging
problem,” he says. “Women are
more likely to get an osteoporotic fracture than breast, ovarian
and uterine cancer combined.”
“Eight to nine years ago,” he
continues, “I thought it would be
a good idea to produce a beverage
to deliver calcium. Since then,
manufacturers have added cal-

cium to everything from orange
juice to chips and crackers.”
The calcium they’re adding is
most often calcium carbonate,
which is insoluble in water and
leaves precipitate at the bottom
of a container. In attempts to
avoid this, Dr. Dickinson bought
chemicals, pH meters and equipment and turned his kitchen into
a chemistry lab.

Dr. Dickinson let his idea and
product hibernate for a couple
of years, until he read a San
Francisco Chronicle article on
Wyck Hay and KaBoom, a new
energy drink Mr. Hay developed
in his Woodside garage.
Dr. Dickinson remembers
approaching Mr. Hay, who had
recently sold KaBoom, with
the idea for a calcium-enriched
beverage. Mr. Hay liked the idea,
and soon, says Dr. Dickinson,
they started working on production strategies.
The recipe and formula were perfected, and then the fun began.
“The first production run was
in the spring of 2007,” says Dr.
Dickinson. “Then, in August, the
product arrived and was first sold
on the Peninsula at Roberts market.” Osteo is now sold at Biachini’s
and Draeger’s as well.
Dr. Dickinson chose Roberts
because of his strong ties to the
market and to Woodside.
“I grew up in Woodside,” he
says. “My father was born there in
1911; I went to Woodside schools;
I was even a bag boy at Roberts in
high school.”
Based on consumer feedback,
Dr. Dickinson and Mr. Hay have
begun a second stage of development. Dr. Dickinson says they are
working on a low-calorie version
of Osteo and a second product
targeted at a younger audience.
“Osteoporosis is thought to be a
problem for older people,” he says,
“but its root is in young people;
over 75 percent of children don’t get
enough calcium. You put bone into
the skeleton until the age of 20 to 21;
after that, it’s a downhill slope.”
