AutoliteRebates.com link.
3. F ill in this mail-in rebate form. Cut out
the Autolite® Spark Plug UPC symbol(s)
from the package(s), circle price(s) paid
on your dated cash register receipt and
mail all items to the address below.
PLEASE RETAIN A COPY OF YOUR CASH
REGISTER RECEIPT AND UPC FOR
PURCHASED ITEM(S) FOR YOUR
RECORDS.
