ALL PROSPECTIVE PROPOSERS:
The following changes, additions, clarifications, and deletions amend the above-captioned
Request for Qualifications, and shall become an integral part of the Proposal Responses and
the subsequent Agreement. Please note the contents herein and affix same to the documents
you have on hand.
Clarifications:
1. The changes to Section 4.0 Section A, “Contents of Qualifications Statement”
paragraphs 8 and 9”, which were included in Addendum No. 1 and issued on
December 5, should be disregarded. The new revisions are reflected in Addendum 2.
2. The changes to Section 4.0 Section B, “Instructions for Submitting a Response Response Submission Format, Section D, No.s 3, 4 and 5”, which were included in
Addendum No. 1 and issued on December 5, should be disregarded. The new revisions
are reflected in Addendum 2.

Is this contract for only one team or firm, or this is for a poll of firms?

The responsive firms will be shortlisted and a minimum of three (3) proposers
will be selected to enter into a contract with the City.

Consultant Evaluation’s for this purpose?

City forms will result in the rejection of the Response as non-responsive.

Can the City please elaborate on what forms our sub consultants need to

provide for the submittal?
Answer
The RFQ forms are for the prime proposer, however, in addition to the sub

consultant resumes, include company licenses and certifications, Business Tax
Receipts and Florida Corporate Registrations. Section 6.3, “Debarment and
Suspension Certificate”, must be completed by all Respondents in the RFQ
submittal.

*Civil Engineering Experience for the Proposer Firm:
Proposer shall complete and submit Form RFQ-PP for this section of its
Response. For each Form RFQ-PP submitted Form RFQ-PP-R must also be
submitted with the Response. Proposer shall only include projects that have
been completed and are comparable to the types of projects to be awarded
under the Agreement. (List a minimum of 3 projects and no more than 5
projects)

Civil Engineering Projects Experience Past Five (5) Years:
A list of projects of similar size, scope and complexity must be submitted.
Information should include:
* Client’s Name, address, phone number.
* Description of work.
* Year the project was completed.
* Total of fees paid to firm.
* Total cost of the construction, estimated and actual.

*Civil Engineering Experience for the Project Manager:
Provide a comprehensive summary of the Project Manager’s experience related
to Civil engineering. The individual MUST have a minimum of five (5) years’
experience and have served on similar projects on a minimum of three (3)
previous occasions. A list of projects of similar size, scope and complexity must
be submitted. Information should include; client’s name, address, phone
number, description of work, and the year the project was completed. Failure to
meet the five (5) year minimum requirement relative to the Project Manager
will in and of itself result in the proposal being deemed non-responsive.

*Agreement Provisions
Provide comments on, and exceptions to the attached Agreement terms and
conditions. Proposed changes to the Agreement must be returned to the City in
Microsoft Word format with comments reflected by “red-lining” the original
document utilizing the tracking feature. The Microsoft Word document must be
included in the proposal in both printed format and electronically on a CD-ROM.
The City will only consider the identified comments and exceptions during
negotiations. Where a proposal is returned without comments it will be
deemed that the Proposer has no comments or exceptions to the draft
Agreement.
