This Product Safety Alert explains the issue and the steps being taken to rectify the situation. Problem Description Rockwell Automation has determined that a hazard may be created through the improper use and application of the Bulletin 140M Motor Protection Circuit Breakers. PRODUCT SAFETY ALERT Page 2 Such a hazard may be created when a Bulletin 140M Motor Protection Circuit Breaker is improperly adjusted beyond the end of the calibrated portion of the scale (see photo below) and several motor start conditions are attempted in series without any time delay between motor starts. Such simultaneous conditions may cause an internal short circuit to occur within the Bulletin 140M Motor Protection Circuit Breakers, resulting in the creation of a hazard to both personnel and equipment. End of calibrated portion of the scale Un-calibrated portion of the scale, no adjustment settings allowed PRODUCT SAFETY ALERT Page 3 Problem Correction · Effective immediately, all devices listed below under “Product Identification” should have the following field modifications performed. o Inspect all installed product to make sure that the devices are not adjusted beyond the end of the calibrated portion of the adjustment scale. o In the event that a device is adjusted beyond the end of the calibrated portion of the adjustment scale re-adjust the device to be within the calibrated portion of the adjustment scale. o In the event that the installed and properly adjusted circuit breaker does not provide the appropriate overload protection setting, replace the existing device with a new unit that provides the appropriate overload current range. o Add an additional instruction sheet inside the carton boxes of all Bulletin 140M Motor Protection Circuit Breakers that are in inventory. · If there is concern that a user or maintenance person may, in the future, set the dial beyond the calibrated portion of the adjustment scale, then please apply a self-adhesive label near the adjustment scale directly on the Bulletin 140M Motor Protection Circuit Breakers that are either installed or in inventory. This label will clearly show that the device should not be set beyond the calibrated portion of the adjustment scale. Instructions for label installation will be included in the modification kit. · Rockwell Automation will provide customers with a modification kit beginning in January, 2005 (Cat. No. 140M-MK1) that includes the additional instruction sheet as well as the self-adhesive label along with clear instructions on how to apply the label. · Rockwell Automation has modified all present inventories and has implemented changes to the production of frame sizes C, D and F of the Bulletin 140M Motor Protection Circuit Breakers (as well as all starters that use these circuit breakers) to include the above product modifications. · Rockwell Automation will provide customers, upon request, with Anti-Tamper Shields at no charge. The Anti-Tamper Shields (Cat. No. 140M-C-CA) provide protection against inadvertent adjustment of the current setting.
