PROJECT DESCRIPTION - SCOPE OF SERVICE Westbrook High School Roof Restoration Scope of Work The WHS roof restoration project shall consist of a renovation of the existing 92,000 FT² high school roof. The existing roof is made up of a multi-layered modified bitumen asphalt roof with a pea stone over cover. The roof restoration will consist of the following; 1. Removal of the exiting pea stone over cover. 2. Repair and replacement of mod-bit sections as needed based upon infrared scanning. 3. Repair and/or replace all sections of flashings as required 4. Install one new drain in roof area identified in the roof audit with drainage problems 5. Installation of petroleum-based roof coating over the entire high school roof (92,000 FT²) 6. Installation of new pea stone over cover on entire roof Responsibilities of the Selected Firm- The contracting firm’s responsibilities shall include but not be limited to the following: The Contractor shall provide services associated with the contract including, permitting, construction, and warranty. • Develop requirements for safety, quality assurance and schedule adherence • Attend all design review and other special meetings needed • Prepare and submit change order documentation for approval by the Architect and the Owner. • Develop and maintain a method for tracking and expediting review and approvals of shop drawings. • Maintain quality control and ensure conformity to the “highest” intent of the construction documents. • Provide cost control through progress payment review and verification according to approved schedule and contract amounts and terms. 11. Post-Occupancy and Warranty Phase • • Coordinate, monitor and resolve all warranty issues to the satisfaction of CREC two years from substantial completion (beyond where applicable). Develop and maintain an effective program of all Operating Manuals, Maintenance Manuals, and any preventative maintenance data insuring their delivery to the Owner at the completion of the project. Firms are required to submit two (2) copies of their response to the RFP to: CREC Construction Division 108 Charter Oak Avenue Hartford, CT 06106 Proposals are to be clearly identified with the title; Westbrook High School Roof Restoration Project , Attention: Roger LaFleur The proposal must be organized with the following sections: Minimum Requirements – 1. Copy of the firms Connecticut Contractors License. 2. Department of Administrative Services (DAS) pre-qualification certificate indicating pre-qualification in the amount equal to or greater than the project the firm is submitting on. 3. Original signed Insurance Document (see section VIII). Not an insurance certificate. 4. Signed acknowledgement of the firm’s acceptance of the contract terms as identified in Section IV paragraph “Contract” Exhibit H & I. Absence of any of the above minimum requirements will deem the proposer unresponsive. The fee proposal will be returned unopened. VI. GENERAL TERMS AND CONDITIONS A prospective respondent must be willing to adhere to the following terms and conditions and by submitting a proposal hereby accepts and will comply with the terms and conditions of the RFP. 1. Acceptance or Rejection by CREC – CREC reserves the right to accept and/or reject any or all proposals submitted for consideration to serve the best interests of Town of Westbrook. Respondents whose proposals are not accepted will be notified in writing. 2. Ownership of Documents – All documents submitted in response to this RFP are to be the sole property of CREC and the Town of Westbrook. 3. Ownership of Subsequent Products – Any product, whether acceptable or unacceptable, developed under a contract awarded as a result of this RFP is to be the sole property of CREC and the Town of Westbrook unless stated otherwise in the RFP or contract. 4. Timing and Sequence – Timing and sequence of events resulting from this RFP will ultimately be determined by CREC or the Town of Westbrook. 5. Oral Agreements – Any alleged oral agreement or arrangement made by a respondent with any agency or employee will be superseded by the written agreement. 6. Amending or Canceling Requests –CREC reserves the right to amend or cancel this RFP prior to the due date and time, if it is in the best interest of CREC or the Town of Westbrook. 7. Rejection for Default or Misrepresentation – CREC reserves the right to reject the proposal of the respondent that is in default of any prior contract or for misrepresentation. 8. Clerical Errors in Awards – CREC reserves the right to correct inaccurate awards resulting from clerical errors 9. Rejection of Submission – Submissions will be rejected in whole or in part if they limit or modify any of the terms and conditions and/or specifications of the RFP. 10. Changes to Submissions– No additions or changes to the original RFP will be allowed after submittal. 11. Contract Requirements – A formal agreement will be entered into with the firm selected. The contents of the proposal submitted by the successful respondent and the RFP will become part of any contract award. 12. Rights Reserved to CREC – CREC reserves the right to award in part, to reject any and all proposals, in whole or in part, and to waive technical defects, irregularities and omissions if, in its judgment, the best interests of CREC or the Town of Westbrook will be served. 13. Withdrawal of Submission – Negligence on the part of the respondent in preparing the RFP confers no right of withdrawal after the time fixed for the acceptance of the submission. 14. Assigning, Transferring of Agreement – The successful respondent is prohibited from assigning, transferring, conveying, subletting or otherwise disposing of the agreement, its rights, title or interest therein or its power to execute such agreement to any other person, company, or corporation without the prior consent and approval in writing by CREC. The respondent agrees and warrants that in the performance of this contract they will not discriminate or permit discrimination against any person or group of persons on the grounds of race, color, religious creed, age, marital status, national origin, sex, sexual orientation, mental retardation or physical disability, including but not limited to blindness, unless it is shown by such contractor that such disability prevents performance of work involved, in any manner prohibited by the laws of the United States, the State of Connecticut, or the Town in which the school is located . The respondent also agrees to provide CREC’s Affirmative Action Officer with such information that may be requested concerning the employment practices and procedures of the respondent as related to the provisions of this section. 2. The aforesaid provision shall include, but not be limited to, the following: advertising, recruitment, layoff, termination, rates of pay or other forms of compensation, conditions or privileges of employment, selection for apprenticeship, selection or retention of subcontractors, or in the procurement of materials, equipment or services. 3. Nothing herein is intended to relieve any contractor from compliance with all applicable federal, state and municipal legislation or provision concerning equal employment opportunity, affirmative action, non-discrimination and related subjects during the term of its contract on this project. VIII. INSURANCE REQUIREMENTS CREC’s Insurance Requirements Contractors and vendors working for and/or doing business with CREC, shall agree as a condition of acceptance to furnish and maintain, at their own expense, for the duration of any project, work, contract the following policies of insurance covering the following items. Insurance must be primary and endorsed to be noncontributory by CREC, must be written in an insurance company A.M. Best rated as “A-VII” or better, and CREC and the Town of Westbrook must be endorsed to the policy as an additional insured (except Worker’s Compensation) unless specifically waived in writing by CREC. The respondent further agrees that any subcontractor they intend to use on CREC assigned work will be required to submit to the same indemnity and insurance requirements contained in this schedule. Contractor shall obtain insurance certificates stating that both Contractor, CREC and the Town of Westbrook shall be endorsed to the subcontractor’s insurance policies as additional insured (except in the case of workers compensation). Indemnity Clause The respondent, vendor or organization shall save harmless, indemnify, and in the event of either claim notification or suit will immediately defend CREC and the Town of Westbrook and any related or subsidiary entities, their officers, employees and volunteers, from and against all loss, costs, damage, expense, claims or demands arising out of or caused or alleged to have been caused in any manner by the performance of work or use of facilities herein provided, including all suits, claims or actions of every kind or description brought against CREC or the Town of Westbrook either individually or jointly with the Contractor, vendor or organization for or on the account of any damage or injury to any person or persons or property, including the Contractor, vendor or organization’s employees or their property, caused or occasioned, or alleged to have been caused or occasioned in whole or in part by the Contractor, vendor or organization, including any subcontractor, their employees or agents. Certificates of Insurance Before starting any work, the Contractor, vendor or organization shall furnish to CREC a certificate of insurance indicating, specifically, the existence of those coverage’s and limits set forth as follows. CREC and the Town of Westbrook must be named on the insurance certificate as “additional insured ATIMA” for the coverage’s afforded, and a copy of the actual policy endorsement that adds CREC and the Town of Westbrook as an additional insured must be attached to the certificate (Blanket additional insured endorsements are deemed acceptable). It shall be the duty of contractor, vendor or organization to provide such future certificates and endorsements to CREC upon renewal or new placement of any insurance policy which may expire or renew during the term of any project or engagement. Further, that the contractor, vendor or organization shall give CREC thirty (30) days notice of any cancellation or change in the terms of such policy or policies during the periods of coverage. Upon request of CREC, the contractor, vendor or organization shall furnish to CREC for its examination and approval such policies of insurance with all endorsements, or copies thereof, certified by the agent of the insurance company. It is your responsibility to be sure that subcontractors provide acceptable evidence of insurance. CREC also requires that they be named on your general liability policy(ies) as an additional insured. Your general liability policy must be endorsed with ISO Endorsement CG 20 26 07 04 (or equivalent) or ISO Endorsement CG 20 33 07 04 (or equivalent), and ISO Endorsement CG 20 37 07 04 (or equivalent). These form numbers must be specifically referenced on the certificate of insurance. If your insurance company uses another form to provide CREC with additional insured status on your policies, copies of those forms must be provided in advance with the certificate for review and approval by CREC. The proper name for the entities to be named as additional insured is: “Capitol Region Education Council, and/or related or affiliated entities” and the Town Of Westbrook. Evidence of compliance with these requirements is with the ACCORD form 25, “Certificate of Liability Insurance”, with 30 day notice of cancellation, plus copies of any required additional insured endorsements.
