SAFETY - THE CONTRACTOR SHALL CONDUCT HIS ACTIVITIES IN A SAFE MANNER AS SPECIFIED IN THE "CONTRACTOR'S SAFETY REQUIREMENTS" ON THIS SHEET AND IN ACCORDANCE WITH THE AUTHORITY CONSTRUCTION SAFETY MANUAL AND FEDERAL AVIATION ADMINISTRATION (FAA) ADVISORY CIRCULAR 150/5370 "OPERATIONAL SAFETY ON AIRPORTS DURING CONSTRUCTION", LATEST EDITION. NO DESCRIPTION 11. PROTECTION AND REPAIR OF DAMAGE TO EXISTING UTILITIES - THE LOCATION OF THE UNDERGROUND UTILITIES AND FAA CABLES SHOWN ON THE PLANS HAVE BEEN OBTAINED FROM AVAILABLE RECORDS AND FIELD CHECKS AND ARE BELIEVED TO BE CORRECT. CONSTRUCTION LIMITS - ALL CONTRACTOR VEHICLES AND TRAFFIC (UNLESS OTHERWISE AUTHORIZED) SHALL REMAIN WITHIN THE DESIGNATED CONSTRUCTION LIMITS OR HAUL ROUTES. 13. UTILITIES - THE CONTRACTOR SHALL BE RESPONSIBLE FOR PROVIDING PRIOR NOTIFICATION OF HIS CONSTRUCTION ACTIVITY TO ALL UTILITY COMPANIES WHO MIGHT BE AFFECTED. THE CONTRACTOR SHALL MAKE THE NECESSARY ARRANGEMENTS FOR UTILITY LINE STAKING IN ADVANCE OF STARTING THE WORK. CONSTRUCTION SHALL NOT PROCEED WITHOUT PRIOR IDENTIFICATION AND LOCATION OF UTILITIES. 14. THE CONTRACTOR SHALL NOT HAUL ANY SOIL OFF AIRPORT PROPERTY UNTIL THE SOIL HAS BEEN CHARACTERIZED BY THE AUTHORITY'S ENVIRONMENTAL CONSULTANT. THE CONTRACTOR SHALL ALLOW THREE WEEKS FOR THE AUTHORITY TO SAMPLE AND RECEIVE ANALYTICAL RESULTS FROM THE STOCKPILES. 15. ALL EXCESS SOIL IS ASSUMED TO BE CONTAMINATED, BUT NON-HAZARDOUS. THE AUTHORITY WILL CHARACTERIZE THE SOIL FOR DISPOSAL BUT THE CONTRACTOR IS RESPONSIBLE FOR THE STOCKPILING, HAULING, AND DISPOSING OF ALL EXCESS MATERIAL. THE EXCESS SOIL SHALL BE DISPOSED OF AT A RECYCLING FACILITY CAPABLE OF ACCEPTING AND TREATING LOW LEVEL CONTAMINATED SOIL. THE RECYCLING FACILITY SHALL BE APPROVED BY THE AUTHORITY BEFORE THE START OF ANY LAND DISTURBING ACTIVITIES. DISPOSING OF THE SOIL AT A LANDFILL OR RECLAMATION SITE IS PROHIBITED. 16. PRIOR TO STARTING WORK, THE CONTRACTOR SHALL SUBMIT A DEWATERING PLAN FOR REVIEW AND APPROVAL BY THE COTR. AT A MINIMUM, THE PLAN SHALL INCLUDE: A SCHEMATIC DIAGRAM OF THE PROPOSED SYSTEM(S), THE PROPOSED LOCATION(S) OF THE DEWATERING SYSTEM(S), PROPOSED DISCHARGE POINTS, PUMP FLOW RATES, AND PROPOSED SEDIMENT REMOVAL DEVICES. SEDIMENTS SHALL BE REMOVED PRIOR TO DISCHARGING WATER INTO THE AUTHORITY’S STORMWATER SYSTEM. CONTRACTOR'S SAFETY REQUIREMENTS 1. 2. 3. THE CONTRACTOR SHALL MEET WITH THE COTR AND AIRPORT OPERATIONS, AND ACQUAINT ALL OF HIS EMPLOYEES WITH THE AIRPORT ACTIVITY AND OPERATIONS THAT ARE INHERENT TO DCA AND SHALL CONDUCT HIS CONSTRUCTION ACTIVITIES TO CONFORM TO ALL ROUTINE AND EMERGENCY AIR TRAFFIC REQUIREMENTS AND GUIDELINES FOR SAFETY. THIS PROJECT INCLUDES WORK WITHIN THE ACTIVE AIR OPERATIONS AREA (AOA) I.E., THE SECURE PORTION OF THE AIRPORT. ALL CONTRACTOR VEHICLES SHALL BE MARKED AND LIGHTED IN ACCORDANCE WITH DCA REQUIREMENTS. MATERIAL AND EQUIPMENT STORAGE SHALL BE LOCATED AS SHOWN ON THE SITE ACCESS PLAN. MATERIAL AND EQUIPMENT SHALL BE STORED TO PREVENT DEBRIS FROM BLOWING/FLYING AND CAUSING FOD. THE CONTRACTOR SHALL ADVISE ALL MATERIAL SUPPLIERS OF THE DELIVERY ROUTES TO BE USED FOR THE DELIVERY OF EQUIPMENT, MATERIALS, OR SUPPLIES. A REPRESENTATIVE OF THE CONTRACTOR SHALL BE PRESENT FOR DELIVERY OF MATERIALS. MATERIALS AND EQUIPMENT NOT IN USE SHALL BE STORED OR PARKED IN AREAS DESIGNATED BY THE COTR AND OUTSIDE OF THE RIGHT-OF-WAY AND AIRCRAFT OPERATIONAL AREAS AND OBJECT FREE AREAS. THE CONTRACTOR SHALL BE RESPONSIBLE FOR RESTRICTING EQUIPMENT AND PERSONNEL TO THE IMMEDIATE VICINITY OF CONSTRUCTION AND TO AUTHORIZED AREAS AND SHALL TAKE SUCH PRECAUTIONS, AT HIS OWN EXPENSE, AS ARE NECESSARY TO ASSURE COMPLIANCE WITH THESE REQUIREMENTS. 4. 5. MAINTAIN WORK AREAS FREE OF DUST, DIRT AND DEBRIS THAT IN THE OPINION OF THE COTR AND AIRPORT PERSONNEL WILL ADVERSELY AFFECT AIRPORT OPERATIONS. 6. AIRCRAFT TRAFFIC ON RUNWAYS, RAMP AREAS, TAXIWAYS AND APRONS SHALL HAVE PRIORITY OVER CONTRACTOR'S TRAFFIC. THE AIRPORT RESERVES THE RIGHT TO ORDER THE CONTRACTOR AT ANY TIME TO VACATE ANY AREA NECESSARY TO MAINTAIN SAFE AIRCRAFT OPERATIONS. NO ADDITIONAL PAYMENT WILL BE MADE TO THE CONTRACTOR FOR SUCH INTERRUPTIONS TO THE ACTIVITIES. 7. OPEN FLAME, WELDING OR TORCH CUTTING OPERATIONS ARE PROHIBITED UNLESS ADEQUATE FIRE AND SAFETY PRECAUTIONS HAVE BEEN TAKEN AND THE PROCEDURE APPROVED/PERMITTED BY THE DCA FIRE DEPARTMENT. 9. NO EXPLOSIVES OR BURNING OF DEBRIS WILL BE PERMITTED ON THE JOB SITE. NO GUNS, WEAPONS, OR EXPLOSIVES OF ANY TYPE. AIRPORT SECURITY: 1. ALL CONTRACTOR EMPLOYEES SHALL BE BADGED IN ACCORDANCE WITH DCA REQUIREMENTS. 2. VEHICLES OPERATING ON THE AOA WILL BE REQUIRED TO OBTAIN PERMITS IN ACCORDANCE WITH DCA REQUIREMENTS. 3. OPEN TRENCHES, EXCAVATIONS AND STOCKPILED MATERIAL LOCATED IN THE AOA SHALL BE PROMINENTLY MARKED WITH BARRICADES, AND LIGHTED BY FLASHING RED LIGHT UNITS DURING HOURS OF RESTRICTED VISIBILITY AND DARKNESS, AS DIRECTED BY THE COTR AND AIRPORT PERSONNEL. CONTRACTOR WILL INSPECT BARRICADES FOR PROPER OPERATION OF LIGHTS DAILY. 12. MATERIALS AND EQUIPMENT WHEN NOT IN USE SHALL BE PLACED IN APPROVED AREAS WHERE THEY WILL NOT CONSTITUTE A HAZARD TO AIRCRAFT OPERATIONS AND NOT PENETRATE CLEARANCE SURFACES OR OBSTRUCT LINE OF SIGHT. 13. UNSUITABLE MATERIAL AND CONSTRUCTION DEBRIS SHALL BE DISPOSED OF OFF AIRPORT PROPERTY. 14. THE CONTRACTOR SHALL KEEP A LIST OF EQUIPMENT HEIGHTS FOR USE IN DETERMINING CLEARANCES TO AIRPORT PART 77 SURFACES. 15. DEBRIS, WASTE AND LOOSE MATERIAL (INCLUDING DUST AND DIRT) CAPABLE OF CAUSING DAMAGE TO AIRCRAFT LANDING GEAR, PROPELLERS OR BEING INGESTED IN JET ENGINES SHALL NOT BE ALLOWED ON OR ADJACENT TO ACTIVE AIRCRAFT MOVEMENT AREAS. MATERIALS OBSERVED TO BE WITHIN THESE AREAS SHALL BE REMOVED IMMEDIATELY BY THE CONTRACTOR TO THE SATISFACTION OF AIRPORT OPERATIONS. 16. THE AIRPORT OPERATION WILL PROVIDE INSPECTIONS TO RE-OPEN ANY AIRCRAFT AREAS, SUCH AS APRON, TAXIWAY THAT HAS BEEN CLOSED FOR WORK, OR THAT HAS BEEN USED FOR A CROSSING POINT/HAUL ROUTE BY THE CONTRACTOR. 17. THE CONTRACTOR SHALL PROVIDE THE PHONE NUMBERS OF THREE PERSONNEL, INCLUDING THE PROJECT SUPERINTENDENT, WHO MAY BE CONTACTED IN AN EMERGENCY. PERSONNEL SHALL BE ON CALL 24 HOURS PER DAY. 18. ANY SITUATION THAT, IN THE OPINION OF THE AIRPORT PERSONNEL OR COTR CONSTITUTES A HAZARD TO OPERATIONS OF THE AIRPORT SHALL IMMEDIATELY CAUSE WORK AND ACTIVITY TO CEASE UNTIL THE SITUATION IS RECTIFIED. 20. CONSTRUCTION MAY BE SHUT DOWN OR DELAYED TO MAINTAIN SAFETY DURING LOW-VISIBILITY OPERATION. DATE DATE WHERE PROJECT ACCESS REQUIRES TRAVEL ON OR ACROSS RAMP AREAS, TAXIWAYS, OR AIRCRAFT APRONS, THE CONTRACTOR SHALL PROVIDE ADEQUATE PERSONNEL AND EQUIPMENT TO MAINTAIN SUCH SURFACES CLEAR OF DEBRIS. AT A MINIMUM, A REGENERATIVE AIR STREET CLEANING MACHINE SHALL BE PRESENT AT EACH ACTIVE AIRFIELD CROSSING OF A MOVEMENT AREA. AT NO TIME SHALL THE CONTRACTOR'S VEHICLES OR PERSONNEL BE ALLOWED TO ENTER OR CROSS ACTIVE TAXIWAYS WITHOUT PROPER AUTHORIZATION AND ESCORT BY AUTHORIZED PERSONNEL.
