CREDENTIALING REGISTRATION MANAGEMENT SYSTEMS (CRMS) DT PATHWAY III FOR DPD GRADUATES REGISTRATION ELIGIBILITY APPLICATION ON-LINE PROCESS DPD PROGRAM DIRECTOR ON-LINE INSTRUCTIONS IMPLEMENTED: JUNE 2009 – UPDATED: JULY 2014 The Program Director and Student Exit Packets are located on the CDR website at www.cdrnet.org. On the left side of the screen click on Program Directors and then on the next screen under Program Directors click on the DPD Program Director Information for Dietetic Technicians – Pathway III for the Program Director Packet and the Graduating Student Information, which is the Student Exit Packet. Please provide the student copies of the Student Exit Packet or you can direct the student to the CDR website http://cdrnet.org/programdirector/dieteticsprogramstudents.cfm for their graduating student information. DPD Program Director Information for Dietetic Technicians – Pathway III [Program Director Packet] Upon receipt from the student of the DT Registration Eligibility Application for DPD Graduates to sit for the Registration Examination for Dietetic Technicians, verify the original BS degree transcript, the original DPD Verification Statement and the original NDTRE and DTR Mis-Use Form. Following verification on the application please print your name, sign the form and provide your four-digit plus alpha DPD Program Code that designates the DT Pathway III Route. Follow the instructions on the Program Director Student Instructions Checklist for the Registration Eligibility Process. The Program Director Contact Form should be completed and submitted to CDR with the class completion documents. Print and provide for each student, the following forms from the Graduating Student Information (Student Exit Packet) or you can direct the student to the CDR website http://cdrnet.org/programdirector/dieteticsprogramstudents.cfm for their graduating student information. This will assist them with the eligibility examination process. NOTE: The student should retain this form and complete it ONLY if they have a name and/or address change after they have completed and submitted the DT Registration Eligibility Application for DPD Graduates to you for forwarding to CDR. The expiration date appears on the top right hand side of the form. Provide ONLY the Study Guide Order Form for your pathway. CREDENTIALING REGISTRATION MANAGEMENT SYSTEMS (CRMS) DT PATHWAY III FOR DPD GRADUATES REGISTRATION ELIGIBILITY APPLICATION ON-LINE PROCESS Remind the student(s) to complete the online survey located in the center of the DT Registration Eligibility Application for DPD Graduates. Please follow the processing instructions in the Quick Reference Check List – Updated April 2014 for submission to CDR of the following forms: Contact Form, Registration Eligibility Application for DPD Graduates, the Candidate Submission Detail Form and the Class Submission Recap Form. All areas must be completed and dated to ensure prompt processing of the eligibility application(s). Instruct students to keep the Student Copy of the Name Address Change Form for Registration Eligibility in order to submit it to CDR in the future when they have a name and/or address change after they have been processed for the exam (refer to student instructions). Legal documentation must accompany all name changes (i.e., marriage license, divorce decree, court order, etc.). On the CRMS On-Line System enter your Login ID and Password followed by your Program Access Code. [Please note: When there is a Program Director change, the new Program Director must contact Peggy Anderson or Becky Bielinski at CDR to obtain a new Login ID and Password.] The class completion date is the same date the student completed the DPD program and that appears on the DPD Verification Statement. After creating an Eligibility Class Completion Group, proceed to the student screen and enter data from the DT Registration Eligibility Application for DPD Graduates. Student entries should be in Alpha By Last Name Order By Student. If the student is not an Academy member then enter six zeros for the member number. Print each candidate record in Alpha By Last Name Order. Proofread each candidate record, Alpha By Last Name Order, against the DT Registration Eligibility Application for DPD Graduates and edit as required. Keep each student’s records together. Once you have proofread the Candidate Submission Detail form(s) [candidate record(s)] to the DT Registration Eligibility Application for DPD Graduates], and made edits as necessary, click on the “Submit to CDR” button. Submit the following via pdf e-mail to Panderson@eatright.org and bbielinski@eatright.org or send the following original documents by Priority Mail to CDR, attention Peggy Anderson and Becky Bielinski: Program Director Contact Form Class Submission Recap Form [requires Program Director signature] Candidate Submission Detail Form – Alpha By Last Name – of the first student (Printed from candidate screen -- please carefully proofread against the original student application. This is very important because all examination communications will be sent to the address provided on the Candidate Submission Detail Form.) Student’s DT Registration Eligibility Application for DPD Graduates – Alpha By Last Name – of the first student Follow the two previous steps above with the rest of the students in your class – Alpha By Last Name CREDENTIALING REGISTRATION MANAGEMENT SYSTEMS (CRMS) DT PATHWAY III FOR DPD GRADUATES REGISTRATION ELIGIBILITY APPLICATION ON-LINE PROCESS Submit all required class completion documents via e-mail on a pdf file. The e-mail subject line should read: Class Completion {insert the DPD completion date} – DPD {insert four-digit program code, plus a capital T} – {enter #} Students. Retain the originals in your file. (Do Not Type in the parentheses. Include the appropriate wording, spaces and dashes per the example). The pdf document and the e-mail subject line should read the same per the sample below by inserting your respective program code. PLEASE NOTE: Enter your specific four-digit program code following your pathway (i.e. DPD 9112 T, etc). Dashes and appropriate spacing is important for downloading. Class Completion 12-9-13 – DPD 4xxx-9xxx T – 2 Students (T is for technician exam, Pathway III) By labeling the subject line of the e-mail and the pdf file the same, per above sample, it expedites the processing, downloading, filing and tracking of the class. In the event you don’t have pdf access, you are required to submit the Class Completion Packet Via a SECURE AND TRACEABLE ONE- OR TWO-DAY COURIER SERVICE (i.e., PRIORITY MAIL, FEDERAL EXPRESS OR UPS) TO ENSURE DELIVERY. Regular US Mail, Certified Mail or Registered Mail can take up to 7 – 15 days to reach CDR. CDR will acknowledge receipt of your pdf class completion submission via e-mail. Should you not receive acknowledgment within three business days, please e-mail Peggy Anderson and Becky Bielinski at panderson@eatright.org or bbielinski@eatright.org for current update/status of your submission. Program directors will also receive a system generated e-mail once the class has been processed. SPECIAL REMINDERS: Do not submit transcripts and verification statements to CDR unless requested by CDR. Review the Quick Reference Check List to ensure you haven’t missed any of the steps. This will assist them with the eligibility examination process. Remember to provide each student with a minimum of five (5) ACEND Verification Statements if they have used the ones previously provided. CDR only accepts the ACEND Verification Statement (updated 1/12), which is separate and distinct from the DICAS online verification statement for program enrollment.
