Addendum No. 1 – June 21, 2016
The South Windsor Board of Education (SWBOE) is issuing this Addendum to the Request for Proposals namedabove (the “RFP”) to respond to questions and requests for clarifications the BOE received regarding the RFP for
Hazardous Materials Inspection & Sampling, as well as issue additional information and clarifications that shall be
incorporated as part of the RFP.
ITEM 1:
The due date for proposals has been postpone to Tuesday, June 28, 2016 @ 4:00 p.m.
ITEM 2:
Proposer Question: Can we get square feet of existing schools and dates of construction of
original buildings and all additions/portables? Should stress that different building ages
require additional sampling of building materials.
Answer: See information requested below:
1. Eli Terry Building Area: 49,275 SF (Permanent Building, built 1965), 2,500 SF (portables)
2. Philip R. Smith Building Area: 43,382 SF (various years).
3. Note: 3-year AHERA reports were provided at the mandatory pre-bid site visits for both
schools.
4. Eli Terry was built at one time without further additions since the original build in 1965.
During the walk through sampling of all the three different years was discussed. Sampling
required for all three years.
ITEM 3:
Proposer Question: Want to confirm that roof sampling will not be performed at Eli Terry but will be
at Phillip Smith and if so, what areas.
Answer: It is confirmed that no roof samples will be performed at Eli Terry. Provide one roof sample
for each of the year construction areas at Philip R. Smith. If there is more than one area in a build year,
e.g. 1959, take a sample at both areas.
ITEM 4:
Proposer Question: Want to confirm that sampling below terrazzo floors, exterior brick (for each
building age of construction) and below ceramic floors/walls will be completed in limited areas as part
of work.
Answer: This is confirmed. It was discussed that three (3) floor cores and six (6) exterior brick/wall
cores are to be performed at Eli Terry. At Philip R. Smith, provide 4 cores in the 1959 wings (1 in the
front wing and 3 in the main area) and 2 cores in the 1962 areas (2 per area). Respondents shall assume
one sample in the 1988 wing.

ITEM 5:
Proposer Question: Want to confirm bid sheet will include Roofer and Mason for Cuts/Repairs
(contractor to be approved by owner) and will be billed at cost either by consultant or directly by
owner.
Answer: Bid sheet will include roofer and mason for cuts/repairs to be billed through the respondent.
See attached revised sheet. The respondent is permitted to provide a 10% markup on the direct billing
of the contractor(s).
ITEM 6:
Proposer Question: Want to confirm that the bid will not include specifications and abatement
monitoring services. But will include abatement cost estimate.
Answer: This is confirmed that specifications and abatement monitoring services are not included in
this scope. An estimate for abatement cost is required. Bid proposal form has removed the monitoring
services line items.
ITEM 7:
Proposer Question: Want to confirm inspection will include lead XRF screening and if requested 2
TCLP samples.
Answer: Confirmed.
ITEM 8:
All consultants must use a third party testing laboratory. In-house testing will not be permitted.
ITEM 9:
Phase 1 and Phase 2 Environmental Site Assessments are NOT included in this scope of work. A
different RFP is being published for this effort.

Bid Proposal Form
For
South Windsor Phase 2 Early Studies
Hazardous Materials Inspection and Sampling
Lump Sum Fee for Inspection labor to include asbestos, lead,
universal wasted, PCB caulking, glazing and compound sources. Fee
shall include project management and report preparation. For
purposes of bid evaluation, assume the following assumed samples
and provide unit price for each. The lump sum fee proposal shall
include the cost for testing samples. Cost of additional samples will
be reimbursed at the unit rate.
• PLM (180 samples) $___________/sample
