The New York City Home Health Care Division of the Health and Hospitals Corporation has about 200 visiting nurses who follow up visits to patients of the HHC hospital network. At each patient visit, the nurse has to fill out a form with up to 900 questions on it, which up until now has been done entirely on paper. Problem • Nurses would work in the field for a day, then go back to the office in downtown to enter the data into the various backend systems, which creates a lot of inefficiencies Solution • ProntoForms running on Apple iPad2. • The data is linked with HHC's data back end via the Pronto Forms Connector Framework, allowing a secure connection over the internet. Benefits • Current users, typically new employees, experience a marked reduction in the time spent gathering and inputting data. 17 © 2012 AT&T Intellectual Property. All rights reserved.
